The Research Of Easing Stomach Cancer Patients With Preoperative Mild-to-moderate Anxiety By Structuring Effective Coping Style In Therapeutic Communication System Mode

Objectives This study aims at investigating the prevalence and relevant factors of preoperative anxiety of informed stomach cancer patients in general surgery department of third-class hospital of Anhui Province and in the model of therapeutic communication system structuring effective coping style to reduce negative effect for quality of life and surgical treatment and rehabilitation improve the level of surgical preoperative nursing and quality. Methods 232 patients were met the inclusion criteria,and 77 patients anxiety,lastly 49 patients into intervention group(n=26) and control group(n=23)with randomized grouping, those in the intervention group through the model of therapeutic communication system, assessing patients'coping styles and the influencing factors and giving specific solutions,structuring effective coping style in addition routine care,while control group received routine care only. The data collected was analysed by Descriptive statistics, Independent-sample t test, Pearson correlation analysis and Stepwise Regression analysis, Paired t-test for the normal and Wilcoxon rank-sum test for the Non-normal.Results First stage:(1)Preoperative informed stomach cancer patients investigated who have anxiety account to 77,incidence was 33.2%, the preoperative anxiety scores in 44.81±8.55 was higher than the domestic norm(37.23±12.59) (t=5.88,P<0.01);(2)The gender, coping style were influencing factors for preoperative anxiety excepting the age, standard of culture, economic position, vocationa, family address, the mode of payment of medical expenses and primary caregivers.In the Characteristics of coping styles,AC was 34.42±8.14, PC was 29.66±8.20.The Pearson's correlation analysis shows that its were obvious correlation(P<0.01).It was negative correlation with active coping and was positive correlation with passive coping, the correlation coefficient were-0.51 and 0.31.Besides, the incidence rate of preoperative anxiety in male patients was 25.73%,in female patients reached 54.10%.The Wilcoxon rank-sum test shows that it was relevant with gender differences (p<0.01),excepting the age, standard of culture, economic position, vocationa, family address,the mode of payment of medical expenses and primary caregivers(p>0.05);Second stage:the experimental study shows that there were no significant differences in the stomach cancer patients'general information, anxiety and coping styles of the intervention group and control group before the intervention(P>0.05),and there were significant differences after the intervention(P<0.01);there were significant differences in the score of anxiety and coping styles in the intervention group before and after the intervention(P<0.01);there were no significant differences in the score of anxiety and coping styles in the control group before and after the intervention(P>0.05).Conclusions The preoperative anxiety incidence of informed stomach cancer patients was high and clinical nursing workers should give attention. Preoperative anxiety was closely related to coping styles of informed stomach cancer patients.This research suggested:in the model of therapeutic communication system,assessing and structuring patients'effective coping styles was feasible and could effectively relieve preoperative anxiety of informed stomach cancer patients.
