Investigation Of Serum Resistin Level In The Patients Of Essential Hypertension And Atrial Fibrillation

Objective Emerging evidence suggests a role for resistin in inflammation and vascular dysfunction, which may contribute to the pathogenesis of hypertension. We detected serum level of resistin, and examined the association between the serum resistin levels with risk for essential hypertension (EH).Methods Totally 81 subjects including normal blood pressure with sinus rhythm (n=20) and essential hypertension with sinus rhythm (n=31), essential hypertension with atrial fibrillation (n=30) were enrolled in this study. Serum resistin,leucocyte count(WBC),blood cholesterol(TC), tfigeylglyeeml(TG), high-density lipoprotein (HDL-C), low-density lipoprotein (LDL-C),fasting blood glucose(FBG) and high sensitive C-reactive protein (hs-CRP) were examined in each group.Body mass index (BMI) was calculated. Factors that may influence the resistin levels were analyzed. Results (1) Serum resistin (ng/ml) level were higher in overall EH patients than in controls, higher in Af complicating with hypertension patients compared with essential hypertension with sinus rhythm (26.70±0.73 vs 25.06±0.91 vs 22.62±1.35, P<0.05). (2) Pearson correlation analysis showed that resistin was positively correlated with BMI (r=0.2744, P=0.003), SBP(r=0.3426, P=0.018), WBC(r=0.2419, P=0.000), hs-CRP(r=0.2171, P=0.032), left atrial diameter (LA, r=0.4012, P=0.0024), right atrial diameter (RA, r=0.2684, P=0.000). (3) Using Res as the dependent variable seperately, multiple linear regression analysis showed that Res had a linear relation with SBP and hs-CRP. (4) Serum hs-CRP (mg/L) level were higher in overall EH patients than in controls, higher in Af complicating with hypertension patients compared with essential hypertension with sinus rhythm(4.61±1.44 vs 6.04±1.84 vs 1.49±0.87, P < 0.05),but there were not significant difference between two hypertension groups.Conclusion (1)These results showed the phenomenon of Hyperresistinemia in all patients with EH, Res may be recognized as a risk factor and reflected a kind of inflammatory state.(2) Serum resistin level were higher in EH patients complicating with Af compared with EH patients with sinus rhythm, this difference was statistically significance,so the study speculated that when the two kinds of pathological condition coexist , the expression of resistin will not be influenced, and on the other hand, it suggested that there may be a potential link between Res and atrial fibrillation. PartⅡThe Clinical Research of Serum Resistin Level in The Patients of Atrial FibrillationObjective We designed the experiment to detect serum level of resistin and to explore the clinical significance in the Patients of Af.Methods Serum resistin level were detected by ELISA using the American CUSABIO import packaging Elisa KIT in healthy controls(n=20), and Af complicating with hypertension(n=30),lone Af(n=22); And serum level of high sensitive C-reactive protein (hs-CRP) were measured.Results (1)Serum resistin(ng/ml) level were higher in overall Af patients than in controls,higher in Af complicating with hypertension patients compared with lone Af (26.70±0.73 vs 24.10±0.86 vs 22.62±1.35, P<0.05). (2)Meanwhile, we find resistin level were positive correlation with hs-CRP(r=0.4263, P=0.001), SBP(r=0.5886, P=0.000), DBP(r=0.3668, P=0.0007), LAD(r=0.5008, P=0.000), WBC(r=0.3561, P=0.001), monocyte( r=0.3013, P=0.0059); negative correlation with EF(r=-0.3235, P=0.0030). (3)Using Res as the dependent variable separately,multiple linear regression analysis showed that Res had a linear relation with SBP, LAD and hs-CRP, LAD and hs-CRP are independent factors that affected resistin levels.(4)Serum hs-CRP (mg/L) level were higher in Af complicating with hypertension patients than in controls(6.04±1.84 vs 1.49±0.87, P<0.05), higher in Af complicating with hypertension patients compared with lone Af (6.04±1.84 vs 1.72±0.94, P<0.05). But there was no significant difference between lone Af and controls (1.72±0.94 vs 1.49±0.87, P<0.05).Conclusion These results support that resistin level in all patients with atrial fibrillation was increased, it may reflect the inflammatory status in Af patients, participate the atrial structural remodeling, promote the process of atrial fibrillation as an inflammatory factor. To lone Af, resistin perhaps played a role separately, and was more sensitive than hs-CRP.
