Background Lung cancer is one of the most common malignant tumor,and the extreme threat to human survival and health in the current world. In recent years, the morbidity and mortality of lung cancer have been increased obviously in the world. According to our survey results, from 2000 to 2005 ,there were about 120000 new patients of lung cancer in China, among them the male patients were about 70000,increase of 26.9%,and the female were about 50000,increase of 38.4% .The morbidity of female growing faster than male. Besides, the epidemiological data showed that the incidence ratios and clinical characteristics were different with different pathological types of lung cancer, and demonstrates different features with time and area.Objective The aim of this study is to retrospective analyze the distribution of sex, age, smoking , pathology and so on of 1657 lung cancer cases of 8 years from the affiliated hospital of Ningxia Medical University, that help us to understand the incidence and clinical characteristics of lung caner in recent years.Data and Method 1657 inpatient cases of lung cancer in the affiliated hospital of Ningxia Medical University from January, 2001 to December, 2008 were collected. Analyze these cases with Statistical method, describe the comparative characteristics of different pathological types of lung cancer incidence. Results 1. The amount of lung cancer cases in female has increased in the 8 years. The proportion of male and female was 2.67:1.00,among these cases, the proportion of male and female of the former 4 years was 3.10:1.00, and the later 4 years was 2.4:1.00.The cases of male is much more than female in the pathology of squamous cell carcinoma and small lung cancer, but the difference of Adenocarcinoma is small. The squamous carcinoma cases were down,but adenocarcinoma cases were increased markedly in these 8 years.2. Disease-onset age: the mean onset age increased in order as follows, SCLC 57.42 , adenoma 58.41,squamous carcinoma 61.52. The peak age of onset were between the ages of 61-70 of all the different pathological types. The age distribution were mainly about young elderly people.3. The highest rates of smoking was the Squamous cell carcinoma of male, accounting for 89.71%.4. Right lung and left lung of lung cancer incidence ratio was 1.21:1.00,The upper lobe and lower lobe ratio of 1.61:1.00. No correlations were shown among pathological types, incidence of left and right lung cancers, distribution of upper and lower lobes through independent test(P>0.05).5. Clinical manifestation : blood in sputum was easily seen in squamous carcinoma(41.55%), Proliferating type is common in squamous carcinoma with the manifestation of bronchoscope(48.72%).In adenoma, chest and back pains were common symptoms, while in adenoma infiltrating(46.90%)types were common with bronchoscope, and non-abnormal(24.48%)types were second. Irritating dry cough were showed more with SCLC( 70.62%), Proliferating type was also revalent with the manifestation of bronchoscope(46.97%). In imaging, pulmonary atelectasis was common in squamous carcinoma and SCLC, but frequency of spike and pleural effusion was higher in adenoma(24.11%)than that of other groups. 6.SCLC and adenoma obtained cytological evidence mainly with Bronchoalveolar Lavage or brush biopsy .The percentage of getting cytological evidence with pleural effusion in adenoma was higher than that of other types.7. Among the 1473 cases of lung cancer that can clear staged(including clinical staging),there were about 1100 cases over IIIa stage, accounting for 75.36%.Conclusion:1.The epidemiological characteristics of lung cancer had changed significant in the 8 years. The female patients were increased. The incidence of adenocarcinoma had increased in different pathological types.2. The peak age of incidence of lung cancer was 61-70. The age of onset of SCLC and adenocarcinaoma was smaller than squamous cell carcinaoma.3.The different pathological types of lung cancer were male-dominated, but the adenocarcinaoma did not differ greatly between men and women.4.The incidence of squamous cell carcinoma have a larger relationship with smoking than other pathologic types.5. Lung cancer were more likely to happen in the right lung, and more upper lobe than lower, and there have no relationship between the location of the cancer and the different pathology. Adenocarcinoma was more likely to violate two or more parts.6. Different pathological types of lung cancer had different symptoms, imaging, and bronchoscopic characteristics.7.The distant metastasis were more likely to occur in SCLC and adenocarcinoma. Early diagnosis and treatment of lung cancer is still a major challenge.
