GDF-15 Hcy BG And Serumlipid Level Variation And Clinical Significance In CHD

Objective: To determine the variation of GDF-15 Hcy BG lipid level in CHD and explore the clinical significance, and provide evidence for diagnose and prognosis.Methods: The study was collected 108 CHD patients from 2010.4 to 2011.2 admitted to Department of Cardiovascular Disease in Affiliated Hospital of Yanan University, and final diagnosis 102 patients, collected heailth control 20 people who had the same set of sex and age in order to ensure comparability. Two groups collected clinical data with the same method. The SPSS 17.0 was used to analysis the statistical data.By the coronary angiography as a gold standard to analysis validity estimate.Results:1.GDF-15 Hcy BMI were highly elevated than health control group(P<0.05),HDL-C was lower than health control group(P<0.05).2. Hcy in male were highly elevated than femal(eP<0.05),TG,TC Apo-A in female were highly elevated than male(P<0.05).3. The variation of Apo-A in CHD correlated with age(P<0.05),the maximum in 60～74 years.4. the USP pations displayed signficantly higher Apo-A and TC levels as compared with the AMI group(P<0.05), the AMI group displayed higher GDF-15,Hcy,BG than the USP(P<0.05).5.the CHD with abnormal glucose metabolism GDF-15,Hcy was displayed higher than only CHD(P<0.05).6.The serum levels of GDF-15 and BG is positive correlation with the character of coronary artery, The Apo-A is negative correlation with the character of coronary artery.7.Smoking is significance risk factor in females, the rage is 66.57%,Hypertention and fat also were significance risk factor, the rate were 42.09% and 44.76%.To refer to Qian Ming(suggested the cutoff point of GDF-15 was 290.4pg/ml),we think the cutoff point of GDF-15 was 280.4pg/ml,its sensitivity was 62.75%,its specificity was 100%.The sensitivity of electrocardiogram was 80.39%, its specificity was 100%.Both GDF-15 and electrocardiogram were used parallel test, its sensitivity was elevated (88.24%),false negative rate was lower than only lectrocardiogram(8.05%).Conclusions: The pations had worse prognosis if they had accompanied with abnormal glucose metabolism or abnormal serum lipid level or smoking or hypertention or fat etal. GDF-15 was one of prognostic information factors for acute myocardial infarction,it also was had value in severity of vessel, the more importance that the sensitivity was elevated and the false negative rate was lower in diagnosis CHD with GDF-15 and electrocardiogram.
