ObjectiveTo observe the characters of ventricular depolarization of Three-dimensio -nal Electrocardiogram(3D-ECG)on healthy young people,and compared to vectorcardiogram. To provide the basis for clinical application of 3D-ECG.MethodsRecording 12-lead electrocardiogram,vectorcardiogram and 3D-ECG in sequence in 100 males and 100 females whose age are between 20-40. The electrode position of Electrocadiogram(ECG)is Wilson lead-system,the electrode position of 3D-ECG is Frank lead-system. After collecting the datas of ECG,VCG,3D-ECG,selecting the first beginning dot and the last end dot of P-QRS-T waves accurately on X-Y-Z lead ECG,then getting the datas of the maximum vector angle and magnitude of QRS vector loop,the beginning 40 milliseconds vecror parameters of QRS vector loop,the different directions running time of QRS vector loop of three-dimensional Electroca -diogram(3D-ECG). All datas are expressed as means±SD((x|-)±S),SPSS13.0 analysis software is used to calculate the parameters 95% range. After Homogeneity of variance test,all datas take as T-test,between groups take as One-way ANOVA analysis. P <0.05 is taken as statistically significant.Results1,The running time and maximum vector magnitude of QRS vector loop (1)The normal range of running time of male is 80ms~107ms,female is 76ms~108ms; The maximum vector magnitude range of spatial QRS vector loop of male is 0.67mV~2.39mV, and female is 0.72mV~2.02mV, there are obviously different from 3D-ECG in gender(male is greater than female, P<0.05); The position of spatial QRS vector loop all in left-back-down no matter male or female, the normal range of male(elevation range is 20°~86°, aziuth range is -108°~38°), and female(elevation range is 15°~85°, aziuth range is -107°~41°);(2)The maximum vector magnitude of QRS vector loop is obviously different between 3D-ECG and VCG(spatial is greater than VCG, P<0.05). 2,(1) The beginning vector of 10ms of spatial: the vector magnitude is obviously difference in gender from 3D-ECG(P<0.05), the mean vector magnitude of male is greater than female, the normal range of male is 0.001mV~0.12mV, and female is 0.01mV~0.12mV; In spite of male or female, the position of beginning 10ms vector is in right-front or left-front, upwards or downwards, the normal range of male(the elevation range is 42°~144°, the aziuth range is 46°~124°), and female(the elevation range is 51°~141°, the aziuth range is 45°~119°);(2) The beginning vector of 20ms of spatial: the vector magnitude is obviously difference in gender from 3D-ECG(P < 0.05), the mean vector magnitude of male is greater than female. The vector magnitude range of male is 0.02mV~0.46mV, and female is 0.004mV~0.40mV; In spite of male or female, the position of beginning 20ms vector is in left-front or still in right-front, upwards or downwards, the normal range of male(the vector magnitude elevation range is 53°~123°, the vector aziuth range is 35°~117°), and female(the vector magnitude elevation range is 47°~129°, the vector aziuth range is 37°~107°);(3) The beginning vector of 30ms of spatial: the vector magnitude range of male is 0.12mV~0.94mV, and female's 0.10mV~0.88mV; the vector magnitude elevation is obviously difference in gender from 3D-ECG(P<0.05), the normal range of male(the elevation range is 47°~97°, the aziuth range is 5°~103°), and female(the elevation range is 37°~99°, the aziuth range is -2°~92°);(4) The beginning vector of 40ms of spatial: the vector magnitude range of male is 0.42mV~1.52mV, and female is 0.40mV~1.46mV; the vector magnitude elevation is obviously difference in gender from 3D-ECG(P<0.05), the normal range of male(the elevation range is 28°~78°, the aziuth range is -47~67°),and female(the elevation range is 27°~71°, the aziuth range is -46°~60°);(5)In spite of male or female,the spatial-QRS ring vector magnitude of beginning 10ms-30ms is obviously difference from Fp(P<0.05);when 40ms, there is obviously difference compare spatial to Hp and RSp(P<0.05). 3,The main running-time of QRS ring to lie in Left-Front- Down(normal range of male is 5ms~46ms, female is 8ms~40ms) and Left-Back-Down(normal range of male is 4ms~49ms, female is 6ms~53ms).Conclusions1,The maximum vector magnitude of spatial QRS vector loop is greater than(or equle to) each flat QRS loop. 2,The parameters of the maximum vector magnitude of spatial QRS vector loop is simple and without the difference among planes. It is advantager than plane in diagnosis of Ventricular Hypertrop- hy by the changes of the maximum vector magnitude and angle of the spatial vector loop. 3,The spatial vector loop could be observed by any anglepoint,cut pictures in any plane, it is better to observe the true morpha of spatial vector loop; It can avoid the dense of spots for projection by cube spin, this is advanta- ger in diagnosis of Bundle Branch block,W-P-W syndrome and Myocardial infaction than plane, and could make up for shortages of ECG and VCG.
