Objective:To observe the effect of Musk-Earthworm-pill on the pathomorphology of synovium in rabbit with KOA,determine the positive cells of uPA and uPAR in synovium,elucidate the possible basic mechanism of Musk-Aconite-Earthworm-pill on KOA, provide theory basis for clinical application.Methods:1.Divide group and build model:40 healthy male rabbits (weight 2.5Kg±150g) were randomly divided into four groups: A.normal control group, B.model group,C.Musk-Aconite-Earthworm pill group, D.Anti-bone hyperostosis tablet group. Make the OA models by injecting papain to the right knees of experimental rabbits in group B,C and D.2.Give medicine:after the success of mading model,each rabbit of group C was intragastricly administrated Musk-Aconite-Earthworm pill (dissolved in 10ml normal saline) 4 weeks,according to its equivalent dosage/kg/day (6 times of person's dosage /kg/day),1/day; as the same method of group D,which group were given Anti-bone hyperostosis tablet,rabbits of group A and B were given equivalent normal saline 4 weeks.3.Macroscopic observation: Observe the variations of articulus, synovial fluid,cartilage and synovium in each group. Observe the ultrastructure changes of arthrodial synovium of each rabbit,at the same time,evaluate the integral calculus in pathology.4.Observe the expression of uPA and uPAR by immunohistochemistry.Results:1.General conditions: Rabbits of group A were in good condition, activities freedomly; during the experiment, rabbits in modeled group B were lame in right legs,autonomic activities decreased; autonomic activities of rabbits in Group C and D decreased too, after given medicine, activities increased gradually,activities were basically normal at the fourth week. The conditions of eating, drinking and defecation of the four group were normal, body weight of all rabbits increased gradually along with rearing except for group B.2.Macroscopic observation of the articulus, synovia, cartilage and synovium on the whole: there was no swelling and engorgement in knee joints of group A,synovia was clear,cartilage was translucent,smooth and glossy, no hydropsia and engorgement in synovium;right knee joints of group B swelled and engorged obviously,synovia was buff,slightly turbid,quantum increased,cartilage was not glossy,smooth,transparent, coloboma, ulcer and fissure could be seen, bone exposed,damages were severe in condylus medialis tibiae,synovium was obvious hydropsia, engorgement, pachynsis; right knee joints of group C and D swelled and engorged slightly,synovia was clear,slightly buff,quantum decreased compared with group B,cartilage was not glossy,transparency deceased, slight coloboma could be seen,there was no obvious ulcer and bone exposed, the swelling degree of synovium was decreased compared with group B, synovium slightly engorged.3.Observe the changes of pathomorphology in synovium by light microscope:Goup A synovial cell was monostratum,applanation, membrane, semi-transparent film,there was finespun,distribution rule..blood vessel. Group B synovial cell was multichamber, hypertrophy, hyperplasy, there were obvious papillary and blood vessel hyperplasia, inflammatory cells were also seen obviously, mesenchyme was fibrosis. Group C and D synovial cell was hyperplasy,but lesser than group B,there was no inflammatory cells and blood vessel hyperplasia, mesenchyme was fatty tissue,fibrosis was not obvious.The integral calculus, separate for each event and a cumulative total, in pathology of group B increased obviously, they were much higher than group A (P<0.01). In the group with B,C,D and Group C,D both of them achieved statistical significance (P<0.05).4.Expressions of uPA and uPAR: The positive uPA,uPAR located in cytoplasm, brownish or brownish granule. Mainly distributed in synovial lining and synovial below. Compared with group A, expressions of uPA,uPAR in group B increased obviously(P<0.01), compared with group B, uPA,uPAR in group C and D decreased (P<0.05), but the study showed no statistically significant in group C with group D (P>0.05).Conclusion:Musk-Aconite-Earthworm pill can lessen pathomorphological changes of synovium, postpone the degeneration of cartilage, the mechanism is possible related to down regulation the expressions of uPA and uPAR.
