Objective To evaluate the clini-pathological features and prognostic factors of epithelial borderline ovarian tumors.Methods The clinical informations of 71 patients were retrospectively analysed. We compared clinical features between different histologic types. The clinico-pathological characteristics upon recurrence were evaluated by univariate and multivariate analyses.Results The mean age of 71 BOTs was 42.2. The histological types:37 cases were mucinous tumors(52.1%),29 cases were serous tumors (40.9%),5 cases were other types(7.0%). 59 patients(83.1%) were at FIGO stage I ,5 patients (7.0%) at stage II and 7 patients (9.9%) at stage III. It had been found that the tumor size, multilocular cyst, bilaterality, papillary or solid patterns, higher pretreatment CA-125 or CA19-9 level, and microinvasion were significantly difference between serous and mucinous tumors (P<0.05). 39 patients (54.9%) underwent radical surgery and 32 patients (45.1%) underwent fertility-sparing surgery. After surgery, 26 patients received adjuvant chemotherapy. There were 7 relapses, only 3 of which died of disease. 5-year overall survival(OS) and disease free survival rates (DFS) were 97.6% and 96.4%, respectively. Univariate analysis showed that the FIGO stage, microinvasion, peritoneal implants, excisional type(cystectomy or adnexectomy), rupture and bilaterality were signficantly related to recurrence(P<0.05). COX regression analysis indicated that the excision type and rupture were independently prognostic factors (P<0.05).Conclusions BOTs frequently affect younger patients and has an excellent prognosis,while they are not exempted from a risk of recurrence. The FIGO stage, microinvasion, peritoneal implants, excisional type(cystectomy or adnexectomy),rupture and bilaterality were important factors of recurrence and prognosis. Conservative surgery for younger patients with BOTs is safe. Thorough follow-up is warranted ,especially for the patients who have high risks of recurrence.
