Study On The Operating Mechanism Of Dual Referral System Of Community Health Service In Fuzhou City

Research objectives: The dual referral system which, links the basic-level medical institutions and secondary and tertiary medical institutions, is the important constituent in many countries'health care system. Implementing dual referral system has far-reaching meanings on optimizing the health resource distribution, reducing the hospitalization cost, regulating patient's rational flow, and solving the masses progressively the question that"it is to see the doctor expensively, and it is difficult to see the doctor".Because of the lack of effective incentive and restraint mechanisms, which forces the basic medical institutions and comprehensive hospital functions into disorder, and let the patients choose doctors in their minds, and result in the dual referral system non-existing. It is easy to send the patient from community health service centers to hospitals, otherwise it is hard to do so. It is the waste of health resources, because the urban comprehensive hospitals do so many work that should be done in the basic-level medical institutions.Through literature investigation and in-depth interview, this paper gets and analysis the primary materials. Focusing on the diseases which community health centers experience commonly and the diseases which hospitals need to send to community health centers strongly, and according to the predicament of zero downward referral situation with the reason of unbalanced referral system in Fuzhou community health centers, this paper analysis the main inner unbalanced reasons, and gives relative suggestions in order to show some new thoughts and basic information to government to promote the development of community health dual referral. Methods: This paper collects information by literature and in-depth interview.Conclusion:1 Currently, it is hard to set up a well dual referral system between hospitals and community health centers in short time. Fortunately, it is quite realistic in practice to set up the downward referral system which should be put into the single disease clinical pathway, which should be based on the diseases which community health centers experience commonly and the diseases which the hospitals need to send to community health centers strongly, and should be based on the conditions that community health centers should have enough health recourses to do well the work.2 To make the above ideas truly carry out, government plays an important role in realizing the idea which was mentioned above, and government must strengthen intervention, and should balance every stakeholders'benefits. What's more, government should based on the aspects of polices which concludes Health Care System, Chinese Essential Medicine System, and System of Public Hospital to Support Community Health Service, and so on so forth, to promote community health centers to improve their medical conditions to satisfy the patients'needs who come from hospitals. Moreover, hospitals should change their management methods, enhance the hospital bed turnover and encourage more patients to go back to community health service centers to be treated; Finally, the residents should change their referral concepts and behavior.
