BackgroundSepsis is a whole-body inflammatory state which caused by the body overreacting to an infection. Severe sepsis can lead to multiple organ dysfunction syndromes (MODS) which is life-threatening. The pathogenesis of sepsis is complicated. Many theories have highlighted the gut barrier function. The impairment of gut barrier function plays an important role in the development of sepsis. Intestinal is not only the target organ of sepsis, but also the first organ to be affected and spread the inflammatory mediators. How to protect the gut barrier function and reduce the circulation of bacterial endotoxin is an important content in the research of severe sepsis leading to MODS. Currently, western treatments mainly focus on improving the intestinal motility, keeping micro-ecological balance and providing sufficient nutrients to the intestinal mucosa to keep the intestinal function. There is no concept of gastrointestinal dysfunction in Chinese medicine. But according to the syndromes and physical signs, it belongs to constipation, abdominal distension, bloating and nausea etc. According to the syndrome differentiation and multiple ways of drug administration, it has important clinic value to find out the most simple, convent, safe and effective way to improve the gastrointestinal function of severe septic patients.ObjectiveTo evaluate the effectiveness of evodia rutaecarpa and Ground Cloves powder Shenque (CV 8). point application for improving gastrointestinal function of patients with severe sepsis. Scientifically evaluate the safety and effectiveness of point application in improving gastrointestinal function for severe septic patients. Therefore, provide a simple, convenient, safety and efficient clinic method for sepsis patients. Methods60 patients with severe sepsis has gastrointestinal dysfunction were randomly divided into treatment group (n=30) and control group (n=30). According to the Guidelines in 2008, in the control group, patients received antibiotics, fluid resuscitation, vascular active medicine also other support treatments and Chinese medicine, closely observe patient's vital signs, central venous pressure (CVP), urine output and gastrointestinal reaction, bowel sound and abdomen pain. Except the same treatments with control group, patients from treatment group added Shenque point application. It has been applied once a day and can last for twelve hours. After the treatments in both groups, we compared the gastrointestinal function, using APACHEII evaluation, bowel sound, the tolerance of enteral nutrition, intra-abdominal pressure, CRP before and after the treatments by using statistic analysis.ResultsThis research showed significant differences (P<0.05) between the two groups in gastrointestinal function evaluation, bowel sound, intestinal nutrition tolerance rate and APACHEII evaluation, before and after the treatment.ConclusionEvodia rutaecarpa and ground cloves powder Shenque point application is effective in improving severe sepsis patients'gastrointestinal function, bowel sound, APACHEII evaluation and CRP exam, it is also effective in alleviating gastrointestinal reaction and abdominal pressure. Therefore, improve the outcomes of severe septic patients.
