Objective:By doing research on the theory of grading nursing system and demonstrative investigation research, knowing about the paramedic cognition on the implementation and influencing factors of grading nursing system, finding out the problem of its implement, to evaluate the status of the implement of grading nursing system and analysis the influencing factors of implement of grading nursing system. To provide reference for further completing the grading nursing system and put forward referenced solution for putting the grading nursing system in practice more normatively. Meanwhile, to provide guarantee for the development of "High Quality Care Demonstration Project".Methods:1. Literature searchBy using literature retrieval methods, through the CNKI, WANFANG DATA, PUBMED, and MEDLINE, to retrieve domestic and foreign publicly published literature which are relevant to grading nursing system from. On the basis of referring to the related literature and material, to write the theory summarization of grading nursing system and master the domestic and foreign development situation of grading nursing system.2. QuestionnaireMaking Self-designed questionnaires named "Grading nursing actuality questionnaire" (for nurse and for doctor), selecting 450 nurses and 143 doctors from 3A-level hospital in Changchun, I did a survey on the paramedic cognition of the implementation and influencing factors of grading nursing system.3. Statistical methodsUsing Excel official soft ware to input data, and use statistical software SPSS17.0 to make statistical analysis on these data. The statistical method includes descriptive statistical analysis, t-test, chi-square test, multiple regression analysis, etc.Results:1. About the investigating result of the decision-maker of nursing level in nowadays grading nursing system, respondents who chose doctor as the decision-maker of nursing level is 62.2% and 67.8%, respondents who chose doctor and nurse as the decision-maker of nursing level is 36.4% and 28.2%, who chose nurse is 1.4% and 4.0%. More than half of the doctors seldom even never discuss with nurses when they determine the nursing level of patients.2. The cognition point of nurse on grading nursing system is higher than the one of doctor. There are differences on the cognition of grading nursing system between paramedic who is from experimental wards of "High Quality Care Demonstration Project" and others (P <0.05). Cognition point of Paramedic from experimental wards of "quality care demonstration projects" is higher.3. There is statistically significant on the differences of the evaluations on grading nursing system from paramedics. The point of implement of grading nursing system from nurse is higher than doctor. There is statistically significant on the differences of the evaluations on the implement of grading nursing system between paramedic who is from experimental wards of "High Quality Care Demonstration Project" and others (P<0.05) Paramedic from experimental wards of "High Quality Care Demonstration Project" is higher than others.4. There are statistically significant on the differences between doctors and nurses on the cognition of influencing factors of the implement of grading nursing system (P< 0.05).The most important influencing factors are the cognition of grading nursing system, management factors, factors of medicalorder and "High Quality Care Demonstration Project".Conclusion:1. At present, the decision of sufferer's nursing level is still from doctors, there are too few exchanges and cooperation between doctors and nurses.2. There are differences on the cognition of grading nursing system between paramedics. Nurses are better than doctors, paramedics who are from experimental wards of "High Quality Care Demonstration Project" are better than the others3. There is distance between the implement of grading nursing system and its provisions. "High Quality Care Demonstration Project" plays an active role in promoting the implement of grading nursing system.4. There is Linean regression relation among the cognition and implement situation of grading nursing system, management factors, factors of medicalorder and "High Quality Care Demonstration Project". The cognition situation of grading nursing system has the mort influence on the implement of it.
