AIM To observe the effect of artificial liver system to high mobility group box-1 in patients with severe chronic hepatitis B, and discuss the significance of artificial liver system to the treatment of severe hepatitis patients.METHODS There were 41 patients with severe hepatitis B to be selected,32 patients were accepted the treatment of plasma exchange (PE),11 patients were accepted the treatment of continuous veno-venous hemofiltration (CVVH). The contents of HMGB1 was measured by enzyme-linked immunoassay (ELISA) to the 41 patients before the the artificial liver support system (ASLL), and when the treatments ended immediately and 3 days, we compared the diversity of these cytokines levels. The diversity of total bilirubin(TBIL), prothrombin time activity percentage (PTA) were examined.10 normol blood donors were selected for the control group.RESULTS The contents of HMGB1 were increased in severe hepatitis patients significantly to comparison with that in the normal controls (P< 0.01). The contents of HMGB1 before the treatment of PE and CVVH were 11.63±3.59,14.12±2.53 ng/ml. The contents of HMGB 1 after the treatment of PE and CVVH were 7.36±2.78,6.30±.88 ng/ml. There were statistically significant decreased in two group between the lever of the HMGB1 on preoperative and postoperative. There were statistically significant contrast of the clearance for the HMGB 1 between the PE and the CVVH (P< 0.01); The contents of HMGB 1 in the group of the treatment in force were belower than the group of the treatment ineffective, the difference were statistically significant (P<0.01). The contents of HMGB 1 in 3 days after the treatments were rised than the immediately of the treatments, the contrast were statistically significant (P <0.0l), but they were belower than preoperative, the contrast were statistically significant too(P< 0.01).In the research the contents of TBIL were decreased, and the lever of PTA were rised after the PE, the contrast were statistically significant (P<0.01); The contents of TBIL were decreased after the CVVH, the contrast were statistically significant (P< 0.01),but the lever of PTA were not statistically significant between the preoperative and postoperative (P >0.05)CONCLUSION The HMGB1 were eliminated to some extent after the treatment of PE and CVVH. The treatment of ASLL can eliminate the HMGB1, and improve the prognosis of the patients of severe hepatitis B. The ability of the CVVH for clearaning the HMGB1 were better to the PE. The ability of the Artificial liver one time were limited.
