Objectives: Concerning the cardiac function of patients with primary hypertension,we only examined the heart in the human body directly. However,The heart and the arterial system are not separated and their function are depended on each other. Wave intensity is a hemodynamic index,which could be used to evaluate the working condition of the heart interacting with the arterial system.The research objective to observe hemodynamic changes in primary hypertension with wave intensity technology,and the effects of left atrial volume changes on them.Methods:1 62 Patients with essential hypertension were selected from the forth Hospital of Hebei Medical University from Dec.2009 to Feb.2011, and the hypertension patients were divided into two groups, group 2(LAVI <30ml/m2) and group 3(LAVI >30ml/m2). The control group is composed of 30 healthy cases in the same period.2 Carotid arteries of 62 patients with primary hypertension and 32 age-matched normal controls were examined with imaging technique of WI. The following parameters were measured: the first wave peak in early ejection (W1), the second wave peak in late ejection (W2), the negative area during the mid-ejection (NA), the interval between the R wave of the ECG and the first peak of W1 (R- W1), the interval between the first peak and the second peak (W1-W2), the arterial stiffness (β), the elasticity modulus (Ep), Arterial compliance (AC), arterial compliance (AI) and the pulse wave transit velocity (PWVβ). The differences of all the parameters in hypertension and control group were compared.3 By conventional echocardiography, the left atrial volume were detected in the apical four chamber view and two chamber view in left ventricular systolic period when left atrial volume was maximum, The LA volume was corrected for body surface area.4 To analyze the relation of W1, W2, NA, R- W1, W1-W2, LAVI and age, SBP, DBP.5 To explore the correlation of W2 and L AVI.6 The data were analyzed with one-way ANOVA and Pearson.Results:1 W1, W2, NA increased gradually in primary hypertension group of LAVI<30ml/m2 and primary hypertension group of LAVI>30ml/m2. Hypertension group significant increased compared with those of normal controls (P<0.05). There no statistical significances of R- W1, W1-W2 in the three groups (P>0.05).2 Compared with the normal controls, values ofβ, Ep, PWVβwere significantly higher in hypertension group (P<0.05); AC shew low down in the three groups, it was significantly lower in hypertension group with LAVI>30ml/m2 than normal (P<0.05). AI were not related significantly (P>0.05).3 SBP, DBP, Age in primary hypertension group significant increased compared with those of normal controls (P<0.05), There were no significant difference of, Sex, BSA, HR between the hypertension group and the control group (P>0.05).4 W1, W2, NA correlated positively with SBP (r=0.48, P<0.01;r=0.25, P<0.05;r=0.40,P<0.01); NA, W1 correlated positively with age (r=0.28, P<0.01; r=0.23,P<0.05); W2, NA were not related to the DBP; W2 were not related to the age (P>0.05).5 LAVI correlated positively with SBP and age(r=0.26, P<0.05;r=0.32, P<0.01), while no significant related with DBP (P>0.05).6 W2 were not related to the LAVI (P>0.05).Conclusions:1 W1, W2, NA,β, Ep, PWVβin primary hypertension group significance increased compared with those of normal controls (P<0.05), there were no statistical significance between the two hypertension groups.2 WI technology can assess the left ventricular function and reflect the vascular elasticity changes of hypertension patients accurately and objectively. The hemodynamic parameters of WI technology provide a new way to evaluate the dynamics of the heart and vascular system and their interaction.
