Objective: To investigate the clinical characteristics and prognostic factors for women with lung cancer.And provide the reference for clinical treatment and prognosis assess.Methods: A retrospective analysis of clinical datas of 246 women patients with lung cancer and 106 male patients with lung cancer in Fujian Provincial Hospital from January 1,2000 to June 31,2010; And the follow-up visit on prognosis. Using statistical methods to analyze the clinical characteristics and prognostic factors. Kaplan- Meier method was used to analyse the single factor and then Log-rank inspection ; COX model was used to analyse the multiariate factors.Results: Patients ages arranged from 21 to 88 years,an average of 52.1±12.8 years,and a median age of 58 years old. 50.0% (123 / 246) of patients showed cough and expectoration as the first symptom; The misdiagnosis rate was up to 46.0% (113/246), and 60.2% (68/113) of the misdiagnosed patients were misdiagnosed as pulmonary infection; The proportion of positive rate in Bronchoscopy biopsy was 69.2% (101/146), and 34.2% (50/146) in bronchoscopy brush check; Peripheral lung cancer was accounted for 70.0% (172/246); Pathologic types of adenocarcinoma were accounted for 71.5% (176/246), Poorly differentiated cancer was accounted for 49.1% (121/246);Tumor size arranged from 0.6cm to 12.0cm, an average of 4.0±1.8 cm; Advanced cancer (phase III + IV) was accounted for 76.8% (189/246); Lymphnode metastasis patients were accounted for 68.7% (169/246); Distant metastases was accounted for 54.1% (133/246); 76.0% (187/246) patients accepted targeted antineoplastic treatments. The survival time of the patients were from 0.1 to 84.0 months and the median survival time were 16.6 months, 1-year accumulate survival rate were 58.0%, 3-year cumulative survival rates for 22%, and 5-year cumulative survival rates for 11.0%. Kaplan-Meier survival analysis indicated that PS score, tumor differentiation, tumor size, stage of TNM, lymphnode metastasis, distant organ metastases, serum CEA level, surgical resection, targeted antineoplastic therapy were related with prognosis. And tumor differentiation, tumor size, stage of TNM, distant organ metastases, targeted antineoplastic therapy were independent prognostic factors.Conclusion: Most of female patients saw a doctor for respiratory symptoms as cough, chest pain, shortness of breath, haemoptysis etc, the misdiagnosis rate was high. Etiology and Active smoking had not significant correlation with female lung cancer. Adenocarcinoma was the most common pathological type in female with lung cancer. Peripheral lung cancer wae accounted for the most. Female lung cancer had poor tumor differentiation, with strong invasiveness and early metastasis. Most patients were diagnosed as adanced tumors when found. The median oerall surial were 16.6 months in female patients with lung cancer, and were similar to male patients(17.6 months). PS score, tumor differentiation, tumor size, stage of TNM, lymphnode metastasis, distant organ metastases, serum CEA level, surgical resection, targeted antineoplastic therapy were related with prognosis. Tumor differentiation, tumor size, stage of TNM, distant organ metastases, targeted antineoplastic therapy were independent prognostic factors of female patients with lung cancer..
