Objective:Analysis the medial patellofemoral adult normal ligaments and gracilis tendon ,provide reference data for clinical anatomicaly reconstruction of the medial patellofemoral ligament reconstruction.Methods:This cadaver study is based on 30 knees (male15,female 15) . Measure the length of femoral attachment, the position of the patellar attachment ,in longitudinal axis of patella (B/C), medial patellofemoral ligament length,longitudinal axis length (AD); Measure the length and the diameter of gracilis tendon. Measure the width of medial patellofemoral ligament(BC) , estimate the proporton of B and C point in longitudinal axis of patella (AD).Results:1.The medial patellofemoral ligament is a trangular sale-like shape with a proximal and a distal bundle. The femoral insertion origin from adductor tubercle. The patellar insertion locate on the upper half of the medial edge of patella.2.The average length of femoral attachment is 0.7687±0.02924cm to male , 0.6460±0.03397cm to female. The position of B point is about (22.66±0.13)% of patella longtitudinal axis, the position of C point is about (55.67±0.56)% of patella longtitudinal axis. the average length of medialt patellofemoral ligamen is 5.16 cm to male, 4.36 to female .3.The average length of available gracilis tendon is 28.9287±0.67800cm to male , 26.9240±0.45247 cm to female.The average diameter is 0.3820±0.02077cm to male, 0. 3193±0.02915 cm to female. Conclusion:1.The medial patellofemoral ligament is a trangular sale-like shape with a proximal and a distal bundle. The patellar attachment extends from (22.66±0.13)% to (55.67±0.56)% of patella longtitude. The femoral insertion area is found in the adductor tubercle.2.The gracilis tenden average length is 28.9287±0.67800cm (male), 26.9240±0.45247cm (female).Doulde bundle reconstruction of medial patellofemoral ligament by using a free gracilis tendon in an accepted technique.3.In clinic practices, double bundle reconstruction enable to restroe the anatomical feature and the function of the native medial patellofemoral ligament, it show excellent result.
