The Application Of Chinese Medicine Theory Of Constitution To Diagnose And Cure In Common Gynecologic Chronic Pelvic Pain Disease

ObjectiveTo investigate the constitutional patterns of chronic pelvic pain(CPP) and study their characteristics of body constitution, evaluate curative effect on the treatment of chronic pelvic pain and effect to improve quality of living by adopting recuperative medical care of traditional Chinese medicine. The purpose is to giving a new method to treat the disease and enhance quality of life.MethodsAccording to the body constitution theory of traditional Chinese medicine and the diagnostic code of CPP from obstetrics and gynecology of TCM (published by People's Medical Publishing House on 2001),85 cases of chronic pelvic pain are investigated, who are puzzled by CPP and come to the gynecological outpatient clinic of the Guangdong Hospital of Chinese medicine from 2009 to 2011, including the items of age, course of disease, body constitution type, degree of pain, quality of life etc. They are divided into the treated group with 44 cases and the control group with 41 cases stochastically. The treated group are gived the treatment of the combined therapy of by adopting recuperative medical care of traditional Chinese medicine and determination of treatment based in pathogenesis obtained throu-gh differentiation of symptoms and signs, while the control group just are gived the determination of treatment based in pathogenesis obtained throu-gh differentiation of symptoms and signs. At the end of the third month after the treatment began, we let the patients do the short-form McGill pain questionnaire, WHOQOL-BREF again to get the curative effect they feel, the change of their degree of pain and quality of life. The results are analyzed statistically and showed in figures.Results 1.73 CPP patients get body constitution type of TCM voluntarily.15 of them are the constitution of mild, while others are partial constitution. 58 cases of partial constitution is divided into two parts:simple partial constitution(34 cases) and composite partial constitution(14 cases).20.5% cases are the constitution of Yang deficiency(15 cases), and 15.1% are the constitution of Qi depression(11 cases). The constitution of blood stasis(7 cases) and the constitution of dampenss and heat (6 cases) are also common.5.5% cases of composite constitution are the constitution of Qi deficiency and dampenss and heat (4 cases).4 cases are lost during the observation both in the treated group and the control group. The effective power of the treated group is 82.5% while it is 73% in the control group.2.4 cases of treated group are lost during the observation both in the treated group and the control group. After the treatment, we set 4 classifications of curative effect, which included, recovery, excellence, improve, unavailability, then we let the patients choose one as the appreciation of the treatment. The total effective rate of treated group is 82.5%, and the total effective rate of control group is 73%. Compared with two groups in this aspect, there is statistics significance difference (P< 0.05). So the appreciation of the treatment in the treated group is higher than the control group.3. About the quality of life, both two groups have the boost in the grading of the WHOQOL-BREF after the treatments. In the group own comparison, the treatded group has statistics significance difference (P<0.05) in all aspects, and the control group also has statistics significance difference (P <0.05) in all aspects except SOCIL. Between two groups comparison of the phase contrast in subjective reception of health and quality of live,PHYS, PSYCH which both have statistics significance difference (P<0.05), but in the opposite direction there is not statistics significance difference (P>0.05) in ENVIR. So the treated group is better than the control group in improving the quality of live.ConclusionThere are more patients who are partial constitution than that are the constitution of mild. The constitution of Yang deficiency and Qi depression type took up most of cases, especially Yang deficiency is the most. The constitution of blood stasis and the constitution of dampenss and heat are also common. We lady down the abnormal constitution the coresponding strategy which complied with traditional Chinese medicine principal to prevent and treat and to find a new way to prevent and treat CPP.
