Objective:To determine the effect of the completely parenteral nutrition and the combined application of enteral and parenteral nutrition on the therapeutic effect in the patients with serve acute pancreatitis.Methods:In severe acute pancreatitis 144 patients, including male 88 cases, female 56 cases, average age (51.3±13.6) years. According to the different methods of nutritional support cases, divided into total parenteral nutrition (TPN) group (n=68) and enteral nutrition and parenteral nutrition (PN+EN) group (n=76).Compare the situation of the two group of pations such an improved clinical index, change, complications, secondary infection rate, hospitalization days of differences.Results:1.Two weeks after nutritional support, the APACHEⅡscore, ALT, AST and serum amylase in both two groups significantly decreased(P<0.01), while the blood calcium, serum albumin, and total protein significantly increased (P<0.01), however, the blood glucose and serum creatinine had no significantly difference between these two groups(P>0.05).2.After nutritional support, the APACHEⅡscore were significantly lower in the PN+EN group than in the TPN group (P<0.01); the blood glucose, ALT, AST and serum amylase were lower in the PN+EN group than in the TPN group (P<0.05);the blood calcium, serum albumin, and total protein were higher in the PN+EN group than in the TPN group (P<0.05).3.After nutritional support, the course of disease, the time of hospitalization and double infection rates in both two groups significantly decreased (P<0.05).4.The complications were significantly lower in the PN+EN group than in the TPN group (P< 0.01); the average hospital days were significantly lower in the PN+EN group than in the TPN group (P<0.01); the alleviate time of the abdominal distension and temperature after often were more early in the PN+EN group than in the TPN group (P<0.01).Conclusion:The combined application of EN+PN nutrition support and total parenteral nutrition can remarkably improve the nutritional status of patients with severe acute pancreatitis. The combined application of enteral and parenteral nutrition were more superior than total parenteral nutrition.
