| Objectives:To compare the effcets of different nutritional supports on the postoperative clinical outcomes of advanced gastric carcinoma patients and to find the best nutritional support for them.Methords:Retrospective analysis from January 2007 to September 2010 in the first hospital of Shanxi Medical University a total of 226 cases of gastric cancer patients with information. The inclusion of 124 cases were divided into two groups according to nutritional approach:①EN and PN group:53cases;②PN group:71cases.Collect the report of ALT, AST, STB, TB, ALB, PA, HGB, LYM, LYM% of the first day after surgery and the eighth day after surgrey. Observe the time periods for gastrointestinal function retrieved, postoperative hospital stay and the incidence of complications(wound infection, lung infection, gastrointestinal emptying disorders, hyperglycaemia), compare record data between the two groups and within groups.Results:①Before nutritional support, the reports of TP, ALB, PA, HGB, don't have any statistical differences between the two group (p<0.05); After nutritional support, these three indexes all increased and EN & PN group is higher than PN group. ALB and PA have statistical significance in two groups (p<0.05).②Before nutritional support, which is the first day after surgery, the reports of STB, AST, ALT don't have any statistical differences between the two groups (p>0.05). However, after nutritional support, ALT increased in PN group and decreased in EN and PN group; STB, AST increased in both groups, but PN group was more obvious than EN and PN group; There was statistically in PN group (p<0.05), but not so in EN and PN group (p>0.05). STB, ALT have statistical significance between two groups (p<0.05), AST has no statistical significance (p>0.05).③Before nutritional support, the reports of LYM, LYM% don't have any statistical differences between the two groups (p>0.05). After nutritional support, there two indexes both increased and EN & PN group is higher than PN group, LYM has no statistical difference in PN group (p>0.05) but has in EN and PN group (p>0.05); differences between the two group does not have statistical significance (p>0.05). LYM% has differences in and between the two groups, which is statistically significant (p<0.05).④The time periods for gastrointestine function retrieved have statistical significance {EN and PN group}⑤EN and PN group has less complications than that of PN group which is statistically significant (p<0.05).⑥EN and PN group has less postoperative hospital stay than PN group (p<0.05). Conclusion:Using both EN and PN can help gastrointestine movement and protein synthesis, recover the inmune function. And the occurrence of complications and liver damage are less than using PN alone. |
