The Research Of The Value Of YKL-40 And CA125 In Diagnosis Of Endomerail Carcinoma

Objectives To Explore the value of serum YKL-40 and CA125 in endometrial cancer diagnosis.Methods Twenty-three confirmed cases of endometrial cancer and thirty-one cases of healthy women at The First Affiliated Hospital of Guangxi Medical University from March 2009 to April 2010 were reviewed. Using enzyme-linked immuunosorbent assay(ELISA) to measure serum YKL-40 and Using microparticle enzyme immunoassay(MEIA) to measure serum CA125, We compared the differences between the two groups.Results 1.Mean YKL-40 level was 99.2ug/l(range , ) for healthy subjects,mean YKL-40 level was 170.23μg/l for endometrial cancer of earlier period and 126.04μg/l of advanced stage. Mean serum YKL-40 level for endometrial cancer of earlier period is significantly higher when compared with normal and advanced stage (P<0.05). There was no statistically significant difference between advanced stage of endometrial cancer and healthy subjects(P>0.05).2. Mean serum YKL-40 level was 222.8156μg/l(range,) for endometrial cancer of III grade which is significantly higher when compared with normal(99.2033μg/l),and endometrial cancer of earlier(132.5038μg/l) and advanced period(145.6295μg/l) (P<0.05).3. There was no statistically significant correlation between Mean serum level of CA125 and YKL-40 in endometrial cancer patients.4. The approximate area under the ROC curve for YKL-40 as a diagnostic tool for endometrial cancer was 0.788. At the value of 132.46μg/l,YKL-40 has a sensitivity and specificity of 69.6%and 83.3%,respectively. Positive likelihood ratio,negative likelihood ratio,accuracy,positive predictive value and negative predictive value was 4.17,0.20,75.5 % , 78.3%,73.5%, respectively.5. There was no statistically significant difference between YKL-40,CA125 and pathology in consistency of dignosising of all endometrial cancer(P>0.05). YKL-40 is significantly higher when compared with CA125 in consistenting with pathology in dignosising of endometrial cancer of earlier period.Conclusion The determination of serum YKL-40 level may be a convenient marker in the diagnosis of endometrial cancer , may play a more inportant role than CA125 in the diagnosis of minimal-to-mild endometrial cancer .
