BackgroundEndometrial carcinoma?EC?is a common malignant tumor of the female reproductive system[1].Patients with endometrial cancer are getting younger and younger,which seriously threatens the health and quality of life of women.With the development of society and the improvement of medical level,people's requirements for quality of life are getting higher and higher,which leads to more and more expectations for the treatment of endometrial cancer,which makes the diagnosis and treatment of endometrial cancer become one of the key studies.The evolution of endometrial carcinoma is slow and complex.At present,it is believed that most of Endometrial carcinoma is stimulated by estrogen for a long time without progesterone antagonism.the changes of intima include simple hyperplasia,complex hyperplasia and atypical hyperplasia,on the basis of this,the further carcinogenesis is intimal carcinoma,and then continues to proliferate and metastasis.Therefore,the early diagnosis and treatment of endometrial cancer is of great significance to improve the quality of life and prognosis of patients[2].At present,imaging examination and diagnostic curettage are still important methods for the diagnosis of endometrial carcinoma.with the development of medical technology,the detection of serum tumor markers is more and more used in the diagnosis of endometrial carcinoma[3].Current studies have shown that serum human epididymal protein 4?HE4?,carbohydrate antigen 125?CA125?,and carbohydrate antigen 153?CA153?were significantly increased in patients with endometrial cancer[4].Therefore,people began to explore the relationship between some tumor markers and the progress of endometrial cancer,so as to achieve early diagnosis,treatment of endometrial cancer,reduce mortality and improve prognosis.The purpose of improving the quality of life of women.ObjectiveTo study the value of single and combined detection of serum human epididymal protein 4?HE4?,serum carbohydrate antigen 125?CA125?and serum carbohydrate antigen153?CA153?in the diagnosis of endometrial carcinoma,and to provide a new idea for early diagnosis of endometrial carcinoma.MethodsForty-eight patients with endometrial cancer and 48 healthy women who underwent physical examination in the same clinic in the Hospital of in recent two years were selected as the subjects of the study and set up as the observation group and the control group respectively.The HE4,CA125,CA153 levels of the two groups were compared,and the postoperative pathological diagnosis was used as the gold standard for the diagnosis of endometrial carcinoma.To compare the diagnostic ability of HE4,CA125,CA153 and the combined detection of endometrial carcinoma.?the levels of CA125,CA153 and HE4 were detected by electrochemical luminous method and strictly according to Roche Cobes e411automatic chemiluminescence instrument and matching reagent manual operation?Results?1?The level of serum HE4,CA125,CA153 in the observation group was significantly higher than that in the control group?P<0.01?.?2?The positive rate of serum HE4,CA125 and CA153 in the observation group was84.00%,the positive rate of HE4,CA125 and CA153 was 77.08%,56.25%and 60.42%respectively.The positive rate of serum HE4,CA125 and CA153 in the observation group was significantly higher than that in the control group?P<0.05?.?3?The ROC curve area of serum HE4 was 0.947,P<0.05,the confidence interval was0.889 to 1.000;the ROC curve area of serum CA125 was 0.878,P<0.05,the confidence interval was 0.804 to 0.952;the ROC curve area of serum CA153 was 0.747,P<0.05,the confidence interval was 0.642 to 0.851;HE4,CA125,CA153 diagnosis of endometrial cancer is remarkable,and the diagnostic value is high.Serum HE4 is more accurate for the diagnosis of endometrial carcinoma than CA125,CA153,and serum CA125 for the diagnosis of endometrial carcinoma than CA153,and all three are valuable for the diagnosis of endometrial carcinoma?P<0.05?.The area under the ROC curve is HE4>CA125>CA153.?4?In the observation group The mean of serum HE4 in stage III of FIGO stage increased gradually and the difference was statistically significant?P<0.05?;the mean of serum CA125 in stage III of FIGO stage increased gradually and the difference was statistically significant?P<0.05?;the difference of serum CA153 in stage III of FIGO stage was not statistically significant?P>0.05??5?In the observation group,the patients with endometrial cancer had different pathological grade high differentiation?G1?,middle differentiation?G2?,low differentiation?G3?,and the mean of serum HE4 in the pathological grade G1 and G2 and G3 increased in turn,and the difference was statistically significant?p<0.05?.the mean of serum CA125 in G1 was less than the mean in G2 and G3,and the difference was statistically significant?p<0.05?.There was no significant difference in mean of serum CA153 among G1,G2 and G3groups?P>0.05?Conclusion?1?Serum HE4,CA125,CA153 can be used as serological index to assist in the diagnosis of endometrial carcinoma,among which,the specificity,sensitivity and accuracy of HE4are higher than those of CA125,CA153.as serological index for screening endometrial carcinoma has certain reference value.?2?The levels of serum HE4 and CA125 are correlated with the surgical pathological stages of endometrial carcinoma.suggesting that HE4 and CA125 are associated with the development of endometrial cancer.?3?HE4?CA125 and CA153 combined diagnosis of endometrial carcinoma is more valuable.
