Background: Each year the new appearance of Primary Liver Cancer is about 306000 in China, 300,000 people died of Hepatocellular carcinoma,which is 54.6% of the world liver cancer deaths,the mortality is just less than stomach cancer in the countryside ,is less than lung cancer in the city. Primary Liver Cancer has become a great threat to our country people's health because its short natural survival . High rates of PLC in China is mainly because lots of people infected with hepatitis B virus, 9.09% Chinese was infected with hepatitis B virus. Guangxi is the area of high hepatitis B virus infection, 13% people was infected with hepatitis B virus in GuangXi in the epidemiology investigation in 1992. Accordingly , GuangXi is one of the most highest place of PLC,especially the coutry of FuSui.The traditional epidemiology found the incidence of PLC is relevant with HBV infection , AFB1, drinking,and smoking,et al,there is no more new breakthrough in this field in nearly 10 years. Traditional epidemiology illustrates the relationship between environment and disease,but in fact genetic factors in PLC development process also play an important role. PLC related to many factors and many genes ,the interaction of genetic and environmental factors lead to PLC. Deeply study the various environmental factors'synergy and their relationship to the polymorphism of susceptibility genes,will help us to learn more about the pathogenesis of PLC. The genetic susceptibility research about liver cancer is still in the initial stage at present, some degree of differences is exist because of insufficient Samples , ethnic and regional differences, lack of interaction analysis between genes and environment. Currently PLC genetic susceptibility research most focused on the gene detection and analysis of the cancer tissue and adjacent tissue, lack of detection and control study to the parents who have stable genetic relationship with them. Cancer susceptibility is composed of combination effect caused by a set of genetic polymorphisms, different genes has different effection, haplotype analysis based on multiple SNP is needed. Therefore joint several cancer research institutions and hospital common to collect samples, increase sample size, meta-analysis,are the methods which can solve the problem we faced in the research of PLC susceptibility. With the developing of the medical research, people gradually realize high quality biological sample is an important speed sexual factor to translational medicine research. Abundant and complete biological samples as a precious medical resources,is the guarantee of many medical research work. There is a large population and regional wide, multi-ethnic living in our country,we has the abundant biomedical resources. However, our country tumor specimens library established later due to different concepts and understanding . Establish our own nuclear family sample bank of PLC and the control bank, can provide the resources guarantee to the genetic susceptibility research, can help us catch up with the developed countries in biomedical research, can give reference to establish a national sample bank with our features and suitable for China.GuangXi is a place of multi-ethnic living and high PLC incidence, Establish nuclear family sample bank of PLC and the control bank,can make the best of resources in this area, provide guarantee to the genetic susceptibility research,explore the process of establishing sample bank and management, give reference to establish a national sample bank with much more samples,it is a scientific, practical and creative research item .Objective:Establish nuclear family sample bank of PLC and the control bank, provide samples and data to genetic susceptibility research of PLC, analysis hepatitis b virus infection situation of the nuclear family, help them to prevent PLC by detecting AFP and HBsAg, help them to prevent PLC by detecting AFP and HBsAg, explore the problems exist in the process of establishing sample bank and management.Methods:①The original data collection:Inquire the medical records, communicate with the object who is accord with the condition of nuclear family sample bank of HCC and the control bank, fill in registration form which include HBVtinfection history ,cancer history,home address,contact ways and etc.②Collect blood samples: mobilization the patient and their parents to our hospital for detecting AFP and HBsAg etc, Exsanguinate venous blood 7-8ml of the HCC patients and their parents.③Samples′detecting and handling: 5ml anticoagulation blood separated and -20℃freeze-stored, 2ml noanticoagulation blood was used for detecting AFP and HBsAg,etl.④Using EpiData software register information form and blood numbers, the specialist is responsible for storage of samples, check regularly, record storage time, storage location, etc.Results:①The nuclear family sample bank of PLCwas established: 251 nuclear family sample bank of PLC was established from 2006 to 2010,cncluded complete blood samples and related information.②In our survey, 92.03% propositus of Primary Liver Cancer have a positive HBsAg, which confirmed that the HBV infection and PLC incidence has a close relationship.We found HBV infection is high in the nuclear family sample bank of PLC. The HBsAg positive rate is 44.89%(338/753)in the nuclear family sample bank of PLC, propositus′farthers and mothers HBsAg positive rate are 12.35%(31/251) and 30.29%(76/251).③Survey and analysis to nuclear family sample bank of PLC :in our survey we found 30 nuclear family have PLC family history, which 15 cases are first-degree relatives(patient′s parents and brothers or sisters) ,through testing a new Primary Liver Cancer was diagnosed operated.④Control nuclear family sample bank was established: 168 control nuclear family sample bank was established from 2009 to 2011,cncluded complete blood samples and related information.168 propositus′HBsAg positive rate is 42.86%(72/168),all the members′positive rate is 28.17%(142/504),propositus′farthers and mothers HBsAg positive rate are 17.26%(29/168) and 24.40%(41/168).Conclusion:①Establish nuclear family sample bank of PLC and the control bank,can make the best of resources in this area, provide guarantee to the genetic susceptibility research,make a contribution to conquer PLC.②Whether the nuclear family sample bank of PLC or the control bank, propositus′mothers′infection of HBV is far more than their farthers′. This may be relevant to mother-to-child transmission in HBV epidemiology,which is worth to further research.③First-degree relatives of PLC is tumor high-risk groups, we can improve their awareness of preventing PLC from propaganda and education, through blood screening a new Primary Liver Cancer was diagnosed.so, blood screening and education can play a role to the early detection and early diagnosis and early treatment of PLC.④By exploring the problems exist in the process of establishing nuclear family sample bank and management,our experience are accumulated,which can provide reference to the relevant research.
