BackgroundPremature ventricular beats(PVB),which is one of the most common arrhythmia. Normal people and people with a variety of organic heart disease can occur in patients with premature ventricular beats. The opportunity of that normal people occur premature ventricular beats grows with increasing age. Premature ventricular beats often occurred in patients with organic heart diseases such as coronary heart disease,cardiomyopathies,rheumatic heart disease and patients with mitral valve prolapse.Myocarditis,the ischemia, the oxygen deficit, the anaesthesia and the surgery and so on may cause premature ventricular beats, because of the stimulated cardiac muscle from mechanical, electrical, or chemical stimulation.At present,premature ventricular beats diagnosis and the monitor condition's change mainly rests on patient's clinical symptomst,inspections and so on electrocardiogram and cardiac ultrasound,but not explicit clinical biochemistry target.The research confirmed that plasma BNP level is not only reflected the early heart function suffers injury obvious target, moreover with arrythmia close related,to the arrythmia illness's diagnosis,has the development,and the prognosis has the very big reference value.Therefore extrapolated that BNP can be used as an important biochemical indicators for monitoring patients with premature ventricular beats,it to monitors the condition development,the instruction treatment and the prognosis has the important meaning.ObjectiveThis study was to measure plasma BNP levels of the benign premature ventricular beats(BPVB)group,premature ventricular beats and organic heart disease(PVB and OHDA) group and the normal control group,combined with the results of cardiac ultrasound to explore the premature ventricular beats(PVB) to determine whether patients with organic heart disease(OHDA) combined,and to evaluate the clinical value that assessed of patient's condition,to provide a theoretical basis for guiding treatment and prognosis.MethodsAll cases were selected from the outpatient clinic and inpatient-Department of Emergency,the First Affiliated Hospital of Zhengzhou University from April 2009 to August 2009.By 12-lead ECG examination consistent with premature ventricular beats changes in the characteristics of 109 cases of patients,According to whether the combined organic heart disease(OHDA),is divided into benign premature ventricular beats(BPVB)group 38 cases,premature ventricular beats and organic heart disease (PVB and OHDA)group 71 cases,including coronary heart disease(CHD) 28 cases,dilated cardiomyopathy 15 cases,rheumatic heart disease(RHD) 17 cases,mitral valve prolapse (PMV)11 cases,according to symptoms,signs and echocardiography were excluded cardiac insufficiency. Another 30 cases of healthy subjects as control group. Peripheral serum BNP level was measured by triage BNP test at the time of recruitment.Left ventricular ejection fraction (LVEF) and left ventricular end-diastolic dimension and (LVEDD) were detected by Doppler.Result1.The difference of age,sex and CM-MB was not significant between BPVB group,PVB and OHDA group and the control group.2.The mean level of plamsa BNP was 30.53±5.71 ng/L,54.32±6.46 ng/L and 28.07±4.83 ng/L respectively in BPVB group,PVB and OHDA group and the control group.PVB and OHDA group of plamsa BNP levels were significantly higher than BPVB group and control group (P<0.01),In the patients with BPVB group,the level of BNP was slightly higher than that of the patients with contron group,but no significant difference.3.The LVEDD level was markedly increased in PVB and OHDA group compared with that of BPVB group, control group,50.65±3.88 mm vs 46.92±1.73 mm and 46.71±1.86 mm(P<0.01),but no significant difference was found between BPVB group and control group;The LVEF level was markedly degraded in PVB and OHDA group compared with that of BPVB group,control group,59.24±2.85% vs 62.26±2.19% and 63.14±2.18%(P<0.01),but no significant difference was found between BPVB group and control group. 4.The mean level of plasma BNP was 53.04±6.94 ng/L,56.20±7.37 ng/L, 54.71±6.00 ng/L,and 54.45±4.30 ng/L respectively in PVB and CHD group,PVB and DCM group,PVB and RHD group and PVB and PMV group.There was no significant difference among the groups.5.There was no obvious correlation between the level of plasma BNP and the age (r=-0.073, P>0.05),the level of CM-KB(r=0.151, P>0.05) in PVB and OHDA group.The plasma BNP level was positively correlated with the heart rate (r=0.541, P<0.01) and the LVEDD (r=0.357,P<0.01) in PVB and OHDA group. The plasma BNP level was negatively correlated with the LVEF(r=—0.470, P<0.01)in PVB and OHDA group.Conclusions1.The BNP level was markedly increased in PVB and OHDA group compared with that of BPVB group,control group,but no significant difference was found between BPVB group and control group.Description BNP can be used as a sensitive indicator of PVB to determine whether the combined OHDA.2.The mean level of plasma BNP was markedly increased in PVB and OHDA group,but PVB and OHDA group of plasma BNP levels between groups was no significant difference.Description BNP can be used as a sensitive indicator of PVB to determine whether the combined OHDA,but clinically relevant examination remains to be done to make it clear whitch kind of organic heart disease it is.3.Benign premature ventricular beats group of plasma BNP levels, LVEDD,LVEF than the control group no significant change, indicating that benign premature ventricular beats of the heart structure and function in patients with no obvious change,BNP can be used as an important biochemical indicators for monitoring patients with benign premature ventricular beats,BNP for monitoring the progression of the disease and guide treatment and prognosis of great significance.
