The Comprehensive Evaluation Of The Clinical Efficacy Of Lung Metastases Treated By Body Gamma Knife

Fifty-two patients with 217 lung metastases treated with Body Gamma Knife in department of radiation oncology from June 2007 to December 2008 were analyzed. For body y-knife treatment a stereotactic body frame, for exact tumor localization, reproducible fixation, CT simulation and 3-D treatment plan has been developed. Body y-knife fraction schema-periphery dose 3-5 Gy, per day (50% isodose line),5 days were a week were employed, at a total dose of 30-51Gy and biological effective dose (BED) of 39-86.4Gy. Their clinical data were collected by follow-up investigation and the prognosis was analyzed by regression.The 1-,3-,5-year local control rate were 98.1%,94.8%,94.8%; The 1-,3-,5-year local control rate in the group BED≥75Gy and BED<75Gy were 100% and 96.4%,90.4%,90.4%,respectively. The 1-,3-,5-year overall survival rate were 88.5%,59.6%,41.6%; The 1-,3-,5-year local control rate in the group BED>75Gy and BED <75Gy were 95.8%,77.6%,72.4% and 78.6%,43.9%,22.6%,respectively. The total number of dead cases is 24。There were 9 cases died in respiratory failure,5 cases in multiple organ failure,2 cases in renal failure,congestive heart failure,local recurrence,brain metastases,hemorrhoea, respectively. There was acute radiation injury in upper digestive tract,lung and hematological system. The injury of lung was main problem 6 months after radiation therapy. Side-effect were almost less thanⅡ degree,2 cases with acute radiation injury, and the same to chronic radiation injury.Bodyγ-knife is an effective and safe method of treating pulmonary metastasis, and the dose-time mode is the key.
