Objective: The incidence of pancreatic carcinoma has been rising but our capability of early diagnosis and therapeutic management hasn't obtained the critical progress. Patients with pancreatic cancer have only short survival after diagnosis, irrespective of treatment. Surgery currently provides the best potential for cure in pancreatic cancer. However, this potential must be placed in the joint context of a relative late presentation and the generally poor ability of the patients to withstand radical surgery due to general age and infirmity. Effective palliation requires the sensitive management of the patient's physical symptoms in an integrated management strategy to improve survival and quality of life. Unfortunately, palliation involving courses of chemotherapy or conventional radiotherapy may take 6 weeks to administer. In such circumstances the time commitment and side-effects related to such courses would be likely to dominate remaining survival. Bodyγ-knife is a novel cancer treatment strategy in the late twentieth century practice of clinical radiation oncology of our country.Bodyγ-knife, one strategy of SRT, can be administrated in more higher doses per fraction and reduce toxicity of normal tissues contrast with conventional external beam radiotherapy (EBRT) in which treatment delivery technology a large volumes of normal tissues were exposes to so much radiation each treatment. The aim of this study is to investigate the short-term therapeutic effectiveness and adverse reaction of bodyγ-knife with locally advanced pancreatic carcinoma. Methods: Ninety-three patients with advanced pancreatic carcinoma were treated with OUR-QGD type Stereo tactic Radiotherapy Gamma Rays System. Dose Curve of this group of cases is 50%-65%.Encircled dose of carcinoma which are less than or equal to 5 cm is 3.5-4.5Gy per fraction, of carcinoma which are greater than 5 cm is 3.0-4.0Gy per fraction. The range of total dose is 32-48Gy.Number of treatment is 8-11.Majority of the patients are treated every other day.Results: During the SRT period, symptoms including significant pain,obstructive jaundice and weight loss have been observed relieved (92.4%) and the ratio of digestive tract toxicity and bone marrow depression is 19.3% and 29.5%.After 3 months with SRT, 78 cases are checked by CT again. CR is 8.9%, PR is 35.9%, SD is 43.6%, PD is 11.5%.The level of CEA,CA19-9 has reduced contrast with the level before SRT. No perforation, serious bleeding or continuous high fever has been discovered.Conclusion: Bodyγ-knife is an effective cancer treatment strategy using to be administered to locally advanced pancreatic cancer. Treating Advanced PC with SRT ensures that pancreatic cancer get exact high-dose radiation, with little damage to the normal tissues nearby. SRT has provided clinical radiotherapists with a safe and efficient treatment method. Bodyγ-knife adopting high dose per fraction and short-course of treatment saves time for patients to accept other cancer treatment strategy.
