Objective: To investigate the clinical characteristics and treatment of the elderly with intestinal obstruction, by which it will Provide reference for the diagnosis and treatment of the elderly intestinal obstruction .Methods: The analysis was based on 177 cases of the elderly (>60y) with intestinal obstruction which were collected from January, 2009 to February, 2011.They were analysis comprehensively from the clinical presentation, treatment time, the obstruction causes, the influence of coexisting diseases to diagnosis and treatment, treatment process, treatment outcome, etc ..Results: There were 158 cases we could clear the etiology in the 177 cases , in which there were 139 cases (78.53%) with mechanical ileus,15 cases (8.47%) with dynamic ileus,4 cases with vascular intestinal obstruction. The others were unreasonable. The top three etiology of the mechanical ileuses were intestinal tumor , adhesion and dung stone. They were accounted for 39.55% (70cases) ,23.73%(43cases) and 10.12%(dung stone). In the 70 cases with intestinal obstruction which was resulted in tumor , there were 16 cases (22.85%) of the right-side colon cancer , 45 cases (64.29%) left-side colon and rectal cancer , 3 cases (4.29%) small intestinal tumor , and 6 cases of abdominal metastatic tumor or the other organ's tumor . The clinical time of the 177 cases was in 3 days (42 , 23.73%) , 3 days to 7 days (127 , 71.75%) and for more than 7 days (8 , 4.52%) after intestinal obstruction .There were 122 patients who had one or one more than different coexisting diseases . The morbidity of coexisting diseases was reached to 67.80% . The treatment outcome was bad of the patients who were treated with surgical operation or non-surgical treatment . In the 177 cases , surgical treatment were performed in 78 patients (44.02%) , and non-operated treatment in 99 (55.93%) . Among seventy patients with tumor , 63 underwent operations . 64 patients with primary intestinal tumor , there were 59 performed surgical operation . Among them , 3 small intestinal neoplasm and 56 colon and rectal carcinoma were performed operations . And the three cases with small intestinal tumor received surgical removal of neoplasm . In the 6 cases with abdominal metastasis and other organs'tumor , 2 were performed surgical construction in 4 abdominal widespread metastasis , one case with mesenteric mass was performed ectomy for the mass , and one with ovarian cancer received total bilateral adnexectomy . The treatment effect in operations was better than non-surgical treatment in the intestinal obstruction resulted in tumor . 42 cases with adhesive leus , 32 patients accepted treatment of non-surgical , and the other 10 accepted operative treatment .Among the 177 patients with elderly intestinal obstruction , recovery 126 , improvement 37 , minor improvement 9 and death 5 .Conclusion: 1 . Abdominal carcinoma is the commonest reason leading to the elderly mechanical obstruction . This is followed by adhesive and stone elus . 2 . Immediately performance in elderly intestinal obstruction effect the treatment outcome . 3 . The coexisting diseases are an important affecting factor for treatment outcome . 4 . The age is not the main factor of surgical operation or not . 5 . The surgery should be chosen as the main treatment in the patients with the elderly intestinal obstruction reduced by neoplasm . the elderly patients with adhesive elus can be cured by non-surgery treatment .
