Effect Of Systematic Nursing Intrvention On Survival Quality Of Low-income Patients With Coronary Heart Disease

Objective:Our study was guided by the nursing intervention, conducted through health education and oriented by low-income CHD patients'needs, and we established specific, feasible, clear, interactive nursing intervention process, in the meantime, we carried out systematic and persistent interactive nursing to patients within and outside hospital in order to improve low-income patients' life quality, change their unhealthy lifestyle, alleviate clinical symptoms, reduce the rehospitalization rate, loose the economic burden and reduce the waste of medical resources.Methods:We choosed 120 low-income patients with CHD hospitalized in cardiovascular department of Changchun center university from July to November in 2009, all the patients were randomly assinged into two groups—intervention group and control group. There was no significant difference in age, sex, physical and chemical target, unhealthy life style and the quality of life between the two groups (P>0.05) before intervention.The control group were given ruler nursing, while the intervention group were given systematic and persistent nursing intervention for one year according to "the method of increasing advantage health education contents" and the theory of "knowledge belief and practice".We take Short Form 36 Health Sarvey Questionnaire (SF-36) as the survival quality assessment tools, all the patients'quality of life, physical and chemical target and rehospitalization rate were monitored.Measurement data were indicated by (x±s), and compared by t test,X2 test, and percentage statistical methods to analyze the relationship between the measurement and systematic nursing intervention.Results:1,Because of the low-income, heavy financial burdens and the deterioration of physiological function, the quality of life of low-income patients with CHD were lower than others.2,Systematic nursing intervention could improve patients'quality of life, the SF-36 scores of intervention group were higher than control group, the difference had statistical significance(P<0.05).3.After one year, there are six patients (10%) required readmission in intervention group, and eigheen patients (30%) in control group, so the rehospitalization rate were obviously lower in intervention group compare to control group, the difference had statistical significance(P<0.05).4,Systematic nursing intervention could change unhealthy behavious, after be given intervention, there were 3 patients (5%) keep smoking and 4patients (7%) drinking in intervention group, while there were 10 patients (17%) keep smoking and 13 patients (21%) drinking in control group, the difference had statistical significance(P<0.05). 5,Systematic nursing intervention could enhance the compliance and improve clinical symptoms.After be given intervention, the number of hypertension, hyperlipemia, hyperglycemia and overweight respectively was 2 patients (3%),10 patients (16%),3patients (5%) and 7 patients (12%) in intervention group, while the corresponding number in control group respectively was 11 patients (16%),29 patients (48%),14 patients (23%) and 9 patients (15%), except overweight, the other differences had statistical significance(P<0.05).So patients who were given intervention were quite successful at achieving goals for blood pressure, serum lipids and glucose compare to the control group, but there were no differences in weight.Conclusions:Systematic nursing intervention process was easy, vivid and concree, could improve patients'life quality, alleviate clinical symptoms, reduce the rehospitalization rate and reduce the medical cost.It also can provide references in the part of CHD patients'health education for nurse in basic level.
