The Initial Research Of Nursing Quality Evaluation Index System Of ICU By Using Donabedian's Model

Research purposesThe purpose of this study is to establish evaluation index system of nursing quality of ICU by Donabedian's model, and provide a scientific and effective tool for level evaluation and verti- cal evaluation of nursing quality of ICU.ObjectSelect from clinical nursing management and clinical nursing backbone in ICU from 16 three level hospitals in Shanxi of 30 experts for the study.MethodsApplication of the literature search and interviews to explore evaluation index system based on Donabedian's model ; the application of the method of Delphi expert consultation to screen indicators ; used Analytic Hierarchy Process to determine the weight of indicator; used Cronbach Alphaαto test reliability; used Excell2003,SPSS 17.0 statistical software,Analytic Hierarchy Process yaahp0.5.2 to process data.Results1. Determined structure quality,process quality,outcome quality for the three major categories of indicators and has a layered structure of the evaluation indicators ; the first grade included 3 items,the second grade included 22 items,and the third grade included 71 items; two rounds of the survey of experts assigned that the central tendency of indicators is increasingly obvious and the coefficient of variation is tend to reduce , the coefficient of coordinate is tend to increase, indicating that the views of experts on coordination degree of indicators is gradually increasing ,and the screening of indicators is reasonable.2. Set the weights of various index .Use high and low points difference of the importance of mean difference to indentify Saaty scale, apply Analytic Hierarchy Process to construct judgment matrix, use Analytic Hierarchy Process yaahp0.5.2 software to weight indicators and test index logical consistency. The results showed that indicators at all levels meet the requirements of consistency, the weight of the first grade are : 0.3108,0.4934,0.1958. Combination weights of various indexes are detailed in Appendix 4.3. The choose of expert are reasonable, the findings are credibility. The basic situation of the group of experts to conduct a qualitative analysis, and the authority of the group of experts to conduct a quantitative analysis (the value the authority of expert groups is 0.9235)show that the expert group has good representation, gradation and authority;the consultation questionnaire recovery are 87%,96%, and put forward constructive ideas of exports Accounted for 60%, the rate of adoption is 97.67%, the expert has a high degree of enthusiasm on this study; investigate the coordination coefficients of the overall indicators of experts were0.207,0.272(P<0.01),indicating that coordination of expert is good , therefore, experts forecast are based on practical experience and theoretical basis, the credibility is higher and the findings are desirable.4. Evaluation index system has good reliability. Theα>0.70 through calculating Cronbach Alphaα, the results show that internal of index has good reliability .ConclusionBased on Donabedian's model, nursing quality evaluation index system of ICU including 3 one-level indicators, 22 two-level indicators ,71 three-level indicators,of which the weight of one-level indicators from high to low were : process quality (0.4934),structure quality(0.3108),outcome quality(0.1958)。...
