Objective:To investigate the genetic susceptibility of chronic urticaria. The association of HLA-DRB 1 alleles and serum total IgE with chronic urticaria in Guangxi was evaluated,and the correlation of HLA-DRB 1 alleles with the results of autologous serum skin test in chronic urticaria was analysed.Methods:1. PCR amplification with sequence-specific primers was used to detect HLA-DRB 1 alleles in 144 patients (64 cases of Han and 80 cases of Zhuang) diagnosed with chronic urticaria, the results were compared to those of 199 normal controls(95 cases of Han and 104 cases of Zhuang).2. Autologous serum skin test was carried out on the patient group.According to the results of the test, the patients were divided into two subgroups:the positive group (62 cases) and the negative group(82 cases).The association of HLA-DRB 1 alleles with the results of autologous serum skin test in chronic urticaria was analysed.3. Serum total IgE in the patient group and the normal control group was detected by Western blot.The association of HLA-DRB 1 alleles with serum total IgE was analysed. Results:1.Comparison of the frequency of HLA-DRB1 alleles in Han and Zhuang people:(1) Among the 16 pairs of primer,13 and 16 alleles were detected respectively in Han patients and Han controls,gene frequencies were 0.78%~20.31% and 0.53%~13.68%,respectively.The differences of the frequency of HLA-DRB 1*12,*1401 alleles in the patient group and the control group in Han people were statistically significant(Pc<0.001, RR=6.715; Pc<0.001, RR=28.776).(2) 9 and 14 alleles were detected respectively in Zhuang patients and Zhuang controls,gene frequencies were 0.63%~22.5% and 0.48%~18.27%, respectively.The difference of the frequency of HLA-DRB 1*1401 allele in the patient group and the control group in Zhuang people was statistically significant(Pc=0.002, RR=4.526).(3) HLA-DRB 1*01,*1402 alleles were detected in Han people,while they were not detected in Zhuang people.The differences of frequency of DRB 1*01,*1402 alleles in Han and Zhuang people were not statistically significant(Pc>0.05).HLA-DRB 1*12,*1305 alleles were detected in Han patients, while they were not detected in Zhuang patients.The difference of the frequency of HLA-DRB 1*12 allele in Han and Zhuang patients was statistically significant(Pc<0.001).2.Comparison of frequencies of HLA-DRB 1 alleles in autologous serum skin test positive group,negative group and the control group:The diffences of frequency of HLA-DRB 1*01,*1401,*16 alleles among the three groups were statistically significant(χ2=10.919,Pc=0.032;χ2=35.341, Pc<0.001;χ2=12.694,Pc=0.032).Frequency of HLA-DRB 1*01,*1401,*16 alleles were compared further between all pairwise groups,and differences of the frequency of HLA-DRB 1*1401 allele in autologous serum skin test positive group and the control group,as well as in autologous serum skin test positive group and negative group were statistically significant,respectively (Pc<0.001,RR=17.091;Pc<0.001,RR=7.200).3. Comparison of serum total IgE in the patient group and the control group:The positive rate of serum total IgE in the patient group was 75%,while that of the control group was negetive.The difference of the positive rate of serum total IgE in the patient group and the control group was statistically significant (P<0.001).The positive rate of serum total IgE in autologous serum skin test positive group was 41.9%,while that of autologous serum skin test negative group was all positive.The difference of the positive rate of serum total IgE in autologous serum skin test positive group and negative group was statistically significant(P<0.001).4. The relationship of serum total IgE with HLA-DRB 1*1401 allele:The positive rate of serum total IgE in the patients who were carrying HLA-DRB 1*1401 allele was 51.2%,while that of the patients who were not carrying HLA-DRB 1*1401 allele was 84.5%.The difference of the positive rate of serum total IgE between those two groups was statistically significant(P <0.001).The positive rate of serum total IgE in the autologous serum skin test positive patients who were carrying HLA-DRB1*1401 allele was 35.5%,while that of the autologous serum skin test positive patients who were not carrying HLA-DRB1*1401 allele was 48.4%.The difference of the positive rate of serum total IgE between those two groups was not statistically significant(P>0.05). Conclusion:HLA-DRB 1*12,*1401 alleles may be associated with chronic urticaria in Han people in Guangxi.HLA-DRB 1*1401 allele may be associated with chronic urticaria in Zhuang people in Guangxi.There are discrepancy in the distribution of HLA-DRB 1 alleles in Han and Zhuang people in Guangxi. HLA-DRB1*1401 allele may be associated with autologous serum skin test positive chronic urticaria.The pathogenesis of CU may be the interaction of HLA alleles and environmental factors.
