Objective:To explore the best therapeutic schedule of Standard treatment of Traditional Chinese Medicine(TCM) for Ulcerative colitis(UC), improve the clinical efficacy,and discuss its etiology and pathogenesis by observing clinical efficacy of applying Standard treatment of TCM to cure UC, then to provide clinical basis for further large sample, randomized double-blind clinical study.Methods:1. Understand the Chinese and Western research on the disease through Literature review; 2. Select 66 cases diagnosed as "UC"; 3. Use Chinese medicine Oral+agreed Chinese medicine retention enema+Chinese patent medicine intravenous injection to cure UC basing on syndrome differentiation. After the end of treatment, make a statistics of clinical comprehensive therapeutic effect, TCM curative effects, and the therapeutic effects of main symptoms and mucosal lesions respectively, as well as the TCM symptoms total score before and after treatment Meantime record the clinical adverse events.Results:1. After the treatment, evaluating the clinical comprehensive therapeutic effect, in 66 cases,12 cases (18.2%) reached complete remission,45 cases (68.2%) were effective,9 cases (13.6%) ineffective, and the total effective rate was 86.4%. Evaluating the TCM curative effects,16 cases (24.3%) were completely cured,29 cases (44.0%) had significant effects,20 cases (30.3%) had slight effects, only 1 case (0.5%) had no effect., and the total effective rate was 98.5%. Evaluating the therapeutic effects of main symptoms, the total effective rate of abdominal pain was 92.4%; of diarrhea was 87.9%; of mucus pus and blood was 98.5%; and the total effective rate of tenesmus was 100%.2. Using rank-sum test to analyse the TCM symptoms total score before and after treatment, there was significant difference (P<0.01), which showed a significant improvement in the patients after treatment.3. At the end of treatment, Evaluating the efficacy of mucosal lesions, there were 42 cases (63.6%) in 66 had remarkable effects,18 cases (27.3%) had slight effects,6 cases (9.1%) had no effect. And the total effective rate was 90.9%.4. Adverse reactions:During the period of treatment and after it, no significant adverse reactions were found.Conclusion:1. Standard treatment of TCM is the ideal treatment method to cure UC at present. Not only doed it have unique advantages, but also have an affirmative clinical efficacy; 2. Spleen dysfunction and exuberance of interior dampness is the main pathogenesis of UC in TCM. The syndromes of UC are based on Spleen dysfunction and Kidney deficiency, but presented as damp-heat and blood stasis which appear through all the course of UC; 3. The therapeutic schedule mentioned in the thesis have the roles of tonifying Qi and spleen, clearing heat and dampness,simulating the circulating of blood and converging sore. Its mechanism of curing UC may be related to the following facts:regulating the body's immune function, improving blood circulation, anti-hypercoagulable state, anti-micro-thrombosis, preventing oxygen free radical damage, promoting healing of ulcers, regulating intestinal flora, and so on. The specific mechanism needs further study; 4. Chinese medicine retention enema+Chinese patent medicine intravenous injection is an important method for curing UC; 5. No adverse reactions were found in the therapeutic process.
