ObjectiveUlarative Cotitis,UC also known as chronic nonspecific ulcerative colitis or idiopathic ulcerative colitis,belongs to Chinese "intestinal addiction," "Jiuli," "Jiuxie" and other categaries.It is a chronic non-specific inflammatory bowel disease and the the cause for it has not been made clear yet.The treatment in the modem medicine for the disease mainly depends on anti-inflammatory,immuno-suppression,and support for symptomatic treatment,but the effect was not satisfactory.To enhance the therapeutic effect of the disease,and at the same time to give Chinese medicine full scope to its advantages in the treatment of ulcerative colitis areas,the integration of traditional and western medicine was employed in this research project,in contrast with the Western medicine,so as to compare two methods for the clinical effect of ulcerative colitis.Methods1 Targets in this project were selected from the inpatients in Hubei hospital of Chinese medicine and Wuhan No.1 Hospital between July,2005-July,2007.All cases were confirmed by electronic colonoscopy.According to syndrome differentiation of TCM, all of them belonged to hot and humid connotation and spleen deficiency type. According to the random order by attendance,there were randomly divided into treatment group with 37 menbers and control group with 34 menbers.There were no signifigant differences in the gender composition,age,duration regard between these two groups.2 the treatment group salicylic acid azo sulfasalazine(SASP)were each give to each of the member four times a day and 1g a time,at the same time herbal coloclysis was also carded out each day in the moring and evening repectively according the specificity of each kind of this syndrome,with 100ml/time for 3 treatments each treatment lasts 2 weeks.There is a two-dat stop between the treatment.the control group they were asked to take SASP 4 times a day,1g a time,in addtion,they received the daily enema with 2g SASP dissolved in 200 mL of saline, which lasted for 3 treatments and 2 weeks for each treatment,with stop between each treatment.3 In the course of treatment groups were conducted the effect of observation (observation projects included:signs and symptoms,colon mucosa of the mirror, mucosal biopsy),comparison has been undertaken between the two two groups both before and after the treatment in Chinese medicine symptom total score,posttreatment comprehensive healing efficacy,and the main pathological change of intestinal mucosa after the treatment.Results1 After the treatment,significant differences in the total score of the TCM symptoms can be found in both the treatment group and the control group(P<0.01);After been statistically processed,the treatment group overmatched the control group(p<0.05).2 After the treatments,among 37 cases of the treatment group,17 patients recovered, 10 cases were markedly improved,7 cases improved and 9 cases failed,with the total effective rate of 91.9%.as for the control group with 34 cases,7 recovered,10 markedly improved,10 improved and 7 failed with the total effective rate of 79.4%. After been statistically processed,a significant difference was shown between the two groups,the treatment group overmatching the control group.3 After the treatments,substantial improvement can be found by comparing the intestinal mirror.There are significant difference of the total score between the pretreatment and post treatment(P<0.05);After been statistically processed,the treatment group overmatched the control group.(P<0.05).4 Compared with TCM after the treatments,among 37 cases of the treatment group, 19 patients recovered,9 cases were markedly improved,5 cases improved and 4 cases failed,with the total effective rate of 89.2%.as for the control group with 34 cases,10 recovered,10 markedly improved,6 improved and 8 failed with the total effective rate of 76.5%.After been statistically processed,a significant difference was shown between the two groups,the treatment group overmatching the control group.5 Adverse reactions comparison,among the 34 patients in the control group,12 suffered varying degrees of the side effects,such as nausea,lack of appetite,headache, etc.;among 37 patients in the treatment group 2 patients had mild adverse reactions, e.g.constipation,stomach discomfort.ConclusionThe Chinese medicine believe that this disease locates in intestine,stems in spleen deficiency,appears as hotmess and humidity,which results in a long perriod of complexity of chills and fevers,deficiency and kernel.So the prescription should be established in spleen and stomach,using invigorate thespleen and damp elimination,heat-cleating and detoxicating of chinese crude drug retention-enema, to ensure the medicine can act on the nidus directly,and the medicine can be made full use of,thus promoting inflammation absorption,and improving abnormal metabolism and the local blood circulation,strenthening local immunity,controlling diarrhea and bloody,then to promote the restoration of mucosal lesions,and finally achieve healing.Meanwhile,western medicine,whose high efficientcy is given in to full play,is also employed.The healing effect by integration of traditional and western medicine was much better than the control group while the recurrence rate lower than the control group.The integration of traditional and westem medicine is a significantly effective treatment for ulcerative colitis,and it is recommended to be popularized in clinical use.
