Approaches Of Big Urban Public Hospitals In Supporting Community Medical Service

Objective: In order to further fulfill the medical service demand of the people, implement new medical reform related policies and benefit social and economic development, this article studied the effective approaches of big public hospitals in fully supporting community medical service to improve the ability of community medical service and truly put government's policies, which encouraged public hospitals to support community medical service, into effect. This would overcome the carelessness of policies and avoid them becoming mere formality.Materials and Methods: Totally 30 health clinics or community medical service centers from towns in 12 districts of Zhejiang Province and related community residents were investigated to learn current community medical service situation including land for medical purpose, medical equipment, human resources, medical staff training, community medical service ability and modes, and status of community resident medication. The main service forms and undertaken medical activities from Zhejiang public hospitals in supporting community service were investigated and analyzed.Result: 1. Ambiguity of localization management. 2. Unmatched government'policies. 3. Shortage of community service skill. 4. Insufficiency of personnel and general practitioner training. 5. Unsatisfactory two-way transferring consultation. 6. Deficient policies'and economic guarantee from public hospitals. 7. Unsatisfactory working environment and insufficient personal drive in service. 8. No effective supervision and assessment systems and the service's effect differed from district to district. This article presented current situation of the community medical service and the medication status of the local residents in Zhejiang Province, proposed the main modes for Zhejiang public hospitals to support community medical service and offered some detailed supporting measures which included long-term cooperation agreement signing, personal training, medical guidance and health education, etc. This article also presented some current key problems during provincial public hospitals supporting community medical service. Conclusion: The government is an indispensable driving force in the process of public hospitals supporting community medical service; Public hospitals should be responsible for the social responsibility, which includes personal training, health education and two-way transferring consultation, etc., during the process of the supporting; the long-term cooperative mechanisms should be established which include setting up government, public hospitals and community medical service centers integrated three-level mode; health care service, price system of commonly used drugs and medical insurance integrated reform; general practitioner, clinical training and community practice integrated community medical personal training mode. Governmental supervision and assessment of community medical service should be reinforced. More publicity should be given to form an optimum environment which helps patients see doctors in their own community and only visit big hospitals when their diseases are severe.
