ObjectiveTo investigate the expression and alteration of matrix metalloproteinase-9(MMP-9),neutrophil gelatinase-assoeiated lipocalin(NGAL)in serum and placental tissue in severe preeclampsia(PE), analyze the relationship between MMP-9 and NGAL,discuss their roles in the pathogenesis of PE.MethodsThe levels of MMP-9 and NGAL in serum were detected by enzyme-linked immunosorbent assay(ELISA), the levels of lipid and creatinine(Cr)and Uric acid(UA)were detected with biochemistry analyzer in normal third-trimester pregnant women (including pregnant week≤34W group (n=30) and pregnant week>34W group (n=30)), and in cases of women with early-onset PE(n=30) and with late-onset PE(n=30). The MMP-9 and NGAL mRNA expression levels in placental tissues of women with severe PE and control pregnant women were determined by real-time fluorescent quantitative polymerase chain reaction(real-time PCR).Results①In early-onset and late-onset PE groups, the levels of serum MMP-9 and NGALwere higher than that of their controls, the level in early-onset PE group were higher than that in late-onset PE group (P<0.05in all).②In early-onset and late-onset PE groups, the levels of total cholesterol(TC), triglyeride(TG),low density lipoprotein cholesterol(LDL),apolipoprotein B(ApoB), Cr and UA were higher than that of their controls, while high density lipoprotein cholesterol(HDL) and apolipoprotein A1(ApoA1) were lower than that of their controls (P<0.05 in all).③In early-onset and late-onset PE groups, the serum MMP -9 was positively related to the serum NGAL, MMP-9/NGAL complex, TC, LDL, ApoB and systolic pressure(SP), diastolic pressure(DP), mean arterial blood and pressure(MAP), while the serum MMP-9 was negatively related to the serum HDL(P<0.05 in all).④In early-onset and late-onset PE groups, the serum NGAL was positively related to the MMP-9, MMP-9/NGAL complex, LDL, SP, DP, MAP, Cr and UA while the serum NGAL was negatively related to the serum HDL(P<0.05 in all).⑤MMP-9 mRNA and NGAL mRNA expression were significantly lower in placental tissue in early-onset and late-onset PE groups than control group, and the level in early-onset PE group was lower than that in late-onset PE group(P<0.05 in all).⑥In early-onset , late-onset PE and control groups,placental MMP-9 mRNA expression was positively related to placental NGAL mRNA expression(P<0.05 in all).⑦In early-onset, late-onset PE and control groups the placental MMP-9 mRNA and NGAL mRNA were not associated with serum the MMP-9 and NGAL levels(P>0.05). Conclusions①Increased MMP-9, NGAL and their complex MMP-9/NGAL expression in serum and decreased MMP-9, NGAL placental tissue may be involved in the pathogenesis of preeclampsia, especially in the pathogenesis of early-onset PE.②Decreased expression of MMP-9, NGAL in placental tissue were the one of reasons contributes to impaired invasion of trophoblast cells of PE.③Increased levels of MMP-9, NGAL in serum may be involved in the pathophysiology of vascular endothelial dysfunction led to development of PE.
