Objective To investigate the expression of NGAL and MMP-9in serum in chronic gastritis, intestinal metaplasia, atypical dysplasia and gastric carcinoma, inquire to clinical pathologic characteristic of differentiation, invasion and metastasis of gastric carcinoma, and investigate the expression of Helicobacter pylori(H.pylori), explore their action, relationship in the succession of gastric cancer.Methods To collect the serurm of patients whose pathological results in stomach were one of them:chronic inflammation, intestinal metaplasia, atypical dysplasia, gastric carcinoma. The number of the patients is total88. These patients were divided into4groups, according to gastroscopes situation and pathological results. The patients with chronic inflammation were12, the cases of intestinal metaplasia were26, the cases of atypical dysplasia were27, and the cases of gastric carcinoma were23. The levels of NGAL and MMP-9in serum were detected by enzyme-linked immunosorbent assay(ELISA).The expression of H.pylori were detected by I3C-Urea Breath Test. Distributional characteristics of age and sex of all case groups were analyzed too. The relations with NGAL and MMP-9were taken statistical analysis. The data was analyzed by SPSS17.0.Results1. The composition of sex and age was no significant difference in different groups.2. The serum NGAL levels were18.9±3.6ng/L (chronic inflammation group),19.8±3.5ng/L (intestinal metaplasia group),22.1±3.9ng/L (atypical dysplasia group),25.5±5.1ng/L (gastric carcinoma group) respectively, showing an ascending tendency.3. In gastric carcinoma group, the level of NGAL was higher in low differentiation grade (30.3±5.1ng/L), lymph node metastasis (27.9±4.9ng/L) and Ⅲ-Ⅳstage (28.5±4.4ng/L) than in high differentiation grade (23.7±4.5ng/L), no lymph node metastasis (23.7±4.6ng/L) and Ⅰ~Ⅱ stage (23.6±4.6ng/L) in gastric carcinoma group. Significant difference of the serum NGAL levels were calculated among them statistically.4. The serum MMP-9levels were110.2±62.8ng/L (chronic inflammation group),160.8±82.3ng/L (intestinal metaplasia group),212.9±75.6ng/L (atypical dysplasia group),284.8±89.2ng/L (gastric carcinoma group) respectively, showing an ascending tendency.5. The level of MMP-9was higher in low differentiation grade (452.8±105.8ng/L), Ⅲ-Ⅳstage (367.7±110.2ng/L) and lymph node metastasis (382.6±98.6ng/L) than in high differentiation grade(221.3±99.7ng/L), Ⅰ~Ⅱ stage (231.5±121.0ng/L) and no lymph node metastasis(209.6±96.9ng/L) in gastric carcinoma group.6. The level of NGAL expression was higher in positive group of H.pylori (23.4±4.5ng/L) than in negative group of H.pylori (20.7±7.3ng/L). The level of MMP-9expression was higher in positive group of H.pylori (218.0±102.3ng/L) than in negative group of H.pylori (169.5±79.9ng/L).7. There was a positive correlation between the expression of NGAL and MMP-9in gastric carcinoma (r=0.397).Conclusions1.The excessive expression of NGAL and MMP-9were involved with pathogenesis of gastric mucosa lesion and gastric carcinoma. They were involved with invasion and metastasis of gastric carcinoma. It demonstrated that NGAL and MMP-9could guide early detection and diagnosis of gastric carcinoma, and provide some ideas for the differentiation and stage of gastric carcinoma.2. The expression of NGAL and MMP-9was significantly correlated with differentiation of cancer. TNM stage and lymph node metastasis, but not with age. and gender.3. H. pylori may be one of the factors which induced upregulation of NGAL and MMP-9.4. In summary, NGAL and MMP-9were possible as new targets of early diagnosis and treatment of gastric cancer.
