Purpose:Neutrophil Gelatinase-Associated Lipocalin(NGAL)(also known as lipocalin 2)is a newly discovered pro-inflammatory adipokine,that has been shown to be expressed in patients with coronary heart disease and heart failure Higher than normal population.Essential hypertension is also a disease related to inflammation.This study mainly explored: Whether plasma NGAL is an independent risk predictor of essential hypertension;By exploring the correlation between plasma NGAL and IL-6,whether the increased expression level of plasma NGAL in patients with hypertension is related to the vascular inflammation caused by it;The effect of plasma NGAL on hypertension complicated with neck vascular disease and kidney injury.Methods:Collected the hypertensive patients and non-hypertensive subjects who were hospitalized in the Department of Cardiology of the People's Hospital of Shaanxi Province from November 2017 to November 2019.A total of 308 subjects were studied,and 187 hypertensive patients(86 males,101 females,with an average age of 59.78 ± 10.89 years old),121 gender-age-matched non-hypertensive subjects,all met our inclusion and exclusion criteria.The levels of plasma NGAL and IL-6 were measured by ELISA.Analyze the difference in the expression levels of plasma NGAL between the two groups,analyze their relationship with systolic,diastolic,and mean arterial pressure,analyze their correlation with IL-6,and use the logistic regression model to analyze the correlation between plasma NGAL and hypertension.Use ROC curve of NGAL to predict hypertension.Logistic regression model was used to analyze the correlation between plasma NGAL and hypertension with cervical vascular disease and hypertension with kidney injury,so as to understand whether plasma NGAL has an effect on the occurrence and development of hypertension with cervical vascular disease and hypertension with kidney injury.Results:The two groups were statistically different in age,IL-6,interventricular septum thickness,left ventricular posterior wall thickness,cerebrovascular disease and other factors(P <0.05).The level of plasma NGAL expression in the hypertensive patient group was significantly higher than that in the non-hypertensive subject group(respectively14.97 ± 2.94 ng / ml,12.33 ± 2.88 ng / ml,P <0.001).The correlation between plasma NGAL and systolic blood pressure was r = 0.295,showing a low degree of positive correlation,and the correlation was statistically significant(P <0.001);the correlation with diastolic pressure r = 0.226,showing a low degree of positive correlation,and the correlation had Significant statistical significance(P <0.001);correlation with mean arterial pressure r = 0.280,showing a low degree of positive correlation,and the correlation was statistically significant(P <0.001).Logistic regression model was used to analyze the correlation between plasma NGAL and hypertension,and it was concluded that NGAL is an independent risk predictor of essential hypertension.According to the ROC curve of NGAL predicted hypertension,the maximum Youden index is 0.311,the cutoff value is 13.652,the specificity corresponding to this critical point is 66.90%,the sensitivity is 64.20%;the positive predictive value is 79.20%,The negative predictive value is 48.80%,the area under the ROC curve is 0.728,and P <0.001.The above indicates that the plasma NGAL has a good predictive effect on hypertension.The correlation between NGAL and IL-6,r = 0.272,showing a low positive correlation,and the correlation was statistically significant(P <0.001).Plasma NGAL had no effect on hypertension with cervical vascular disease and hypertension with kidney injury.Conclusion:1.Plasma NGAL was associated with essential hypertension and was an independent risk predictor.2.There is a positive correlation between plasma NGAL and IL-6,suggesting that plasma NGAL can further promote endothelial dysfunction by inducing vascular inflammation,thereby increasing hypertension.
