In the traditional view, atherosclerotic is a progressive linear development, the stenosis and tissue ischemia caused by lipid deposition, which leading to acute cardiac and cerebrovascular events in final, therefore,the focus treatment of atherosclerotic is atherosclerotic plaque regression. But in recent years,the study found that atherosclerotic more than mere lipid lesions in the disease process involved in many inflammatory factors and expression, so it is a chronic inflammatory disease, and the death of atherosclerotic was acute cardiac events have occurred in plaque rupture and thrombosis. The risk of ACS occurrence depends on the stability of plaques rather than the severity of stenosis. NF-κB can regulate the expression of several cytokines, and indirectly promote the expression of CTSD increased, thus promoting the occurrence and development of atherosclerotic.Objective:To detect protein expression of CTSD and NF-κB in normal human coronary artery Pulse group and coronary artery atherosclerosis group by immunohistochemistry.We analyze the relationship between their expression and the degree of atherosclerosis development.We indicate the relationship between inflammation and plaque stability,which can supply experimental evidence for effuture effectiveclinical diagnosis of coronary heart disease, treatment andprognosis evaluation.Methods:Fifty-five left anterior descending coronary arteries from 55 autopsied cases were enrolled.Analyzed by light microscope after the process of HE staining,then the Samples were divided into threen groups stable plaque group(20 samples), unstable plaque group(23 samples) and normal coronary arteries(12 samples).The lipid core size of unstable plaque exceeds 40%of plaque area,while stable group is just the opposite. The exppression of CTSD, NF-κB and CD68 were detected with immuno-histoche-mistory. Nonparametric statistics and Spearmantest are applied in statistics treatment.Result:CTSD protein can be tested in endomembrane of atherscleoro-sis coronary artery,mainly in macrophage,foam cells endothe-lial cell.In control groups,no expression can be detected.There is significant difference between unstable plague and stable plagues(P=0.021, P=0.001) and higher level manifested in unstable ones.NF-κB protein expression can be observed in atherosclerosis,mainly in macrophage,foam cells,endothelial cell, smoothmuscle cell in endomembrane,smooth muscle cell in tunica media Expression difference exists between unstable plague and stable plagues (P=0.000, P=0.002) and higher level manifested in unstable ones. In control groups,no expression can be detected. NF-κB expression level in higher in unstable plagues than that in stable ones.Positive associations between CTSD and NF-κB expression intensity showed in Spearman tests.Conclusion:(1)CTSD and NF-κB expression can be tested in atheros-clerosis and expression level is higher in unstable plagues than stable ones,while no expressions exist in control group.The high expression indicates CTSD and NF-κB are relevant to the unstable plague.(2)NF-κB can regulate the expression of cytokines in atherosclerosis, while cytokines can promote the expression of CTSD.Positive association between CTSD and NF-κB in the region of atherosclerotic plaque indicates that NF-κB is in coordination with CTSD during the development of atherosclerotic plaque.
