Objective:To analyze the Muller classification of distal femoral fractures and understand the choice of treatment according to the classi-ficationMethods:A retrospective study in Second Affiliated Hospital of Dalian Medical University, Department of Orthopedics was carried out in 124 cases(67 males and 57 females,with age range of 16~84 years average 47 years, left legs 72 cases,right legs 52 cases)with distal femur fractures treated from 1999 to 2009. According to the Muller classification. there were 66 cases of type A fractures,23 type B and 35type C. Ten cases(8.1%)were with open fractures.Of all,22 (17.7%)fractures were treated by conservative methods, all of these were belong to Muller A and C.102 by surgical operation, the type B fractures should be treated by suitable fixation method based on the involved areas and comminution degrees,in type A and C 12 were by Less Invasive Stabilization System(LISS),57 were by femoral condylar plates.Results:Two of the cases were not followed up because of theirs.The other cases were followed up for 4-18 months,an average of 9.6 months. The average Neer scores of surgery group was significantly better than that of conservative methods. The average Neer scores of type A,B and C fractures were decreased respectively. The average Neer scores of type A and C were different to adopt different inter-fixations.Conclusions:The surgical operation group is much more better than the conservative group. Less Invasive Stabilization System(LISS) showed good results for distal femoral fractures than femoral condylar plates in type A and C.
