Objective:To perform clinical observations on treatment of twenty-five cases of early diabetic nephropathy with Professor Zhu Zhangzhi's Decent Qi Stenghthening and Evil Removing Prescription by comparison with the control group in clinical therapeutic effects. To analyze the distribution of six channels in the cases of early diabetic nephropathy by undertaking six-channel syndrome differentiation. Methods:Fifty cases of early diabetic nephropathy that met the preset-criteria were selected and divided under the randomized-controlled rule into two equal groups as Traditional Chinese Medicine Group (treatment group) and Control Group. With the same basic diabetes therapies, including nutrition and physical training interference and blood sugar, blood pressure, and blood lipoprotein reduction, etc., the control group was treated with angiotensin II receptor blocker, ARB-valsatan (80-160mg/d according to regular dose), or with angiotensin converting enzyme, ACEI-benazapril (10mg/d according to regular dose), while the treatment group was treated with one dose of Professor Zhu Zhangzhi's Decent Qi Stenghthening and Evil Removing Prescription each day (consisting of DaZao30g,ShuFuZi15g,GanJiang20g,BaiZhu20g,BeiQi45g,GuiZhi15g,MaHuang10g,XiXin10g,BaiShaol5g,ZhiGanCao30g,WuZhuYu9g,YinYangHuo15g,ZeXie15g,FuLing30g,RouGui3g,BianTiaoShenl5g, etc), both groups being treated and observed for eight weeks. Observations on six-channel disease and clinical symptoms of traditional Chinese medicine before and after treatment were performed on each of the fifty patients, whose physiological indices, e. g. blood pressure, blood sugar, urine microproteins, blood creatinine, blood urea nitrogen and BMI, were tested and compared. Results:Under clinical observations, there was no significant difference (P<0.05) in the BMI, blood pressure, blood sugar, and urine microproteins-blood creatinine ratio of the two groups, which indicated comparability between them. In this study, the largest amount of cases of early diabetic nephropathy were classified as shaoyin disease with the method of six-channel syndrome differentiation, taking up 50%of the total cases studied, which showed statistical significance. The Decent Qi Stenghthening and Evil Removing Prescription had a total efficiency of 88.00%in the treatment of early diabetic nephropathy, compared with the 76.00%with the other therapy in the control group. The Decent Qi Stenghthening and Evil Removing Prescription had a total efficiency of 92.00%in reducing the urine microproteins-blood creatinine ratio, with 84.00%in the control group. With no significant difference (P>0.05) found in the above two total efficiencies between the two groups, the two therapies have been proved to be able to effectively reduce systolic blood pressure (P<0.05 before and after treatment) and postprandial blood sugar. Conclusions:Under clinical observations, the largest amount (with statistical significance) of cases of early diabetic nephropathy were classified as shaoyin disease with the method of six-channel syndrome differentiation, which agreed with Professor Zhu Zhangzhi's opinion that the pathogenesis of diabetes is Deficiency of Yang Qi. The Decent Qi Stenghthening and Evil Removing Prescription has had similar effects to ACEI/ARB in staying the progress of diabetic nephropathy by alleviating glomerular filtration rate, reducing postprandial blood sugar, protecting renal function, and decreasing urinary albumin excretion. The Decent Qi Stenghthening and Evil Removing Prescription has been found able to significantly alleviate clinical symptoms in the treatment of early diabetic nephropathy, with a total efficiency of 88%, which indicates that the Prescription has a advantage in improving Traditional Chinese Medicine clinical symptoms and that it is an effective therapy in the treatment of the disease at the early stage.
