Objective:To evaluate the effectiveness and visual quality after combined implantation of refractive and diffractive multifocal lens in both eyes.Methord:This prospective study enrolled 80 eyes of 40 patients divided into multifocal Intraocular lens (MIOL) group(10 patients) and monotifocal Intraocular lens (SIOL) group(30 patients), received phacoemuisification . MIOL group were bilaterally implanted with either the Rezoom NXG1 or the Tecnis ZMA00 multifocal IOL and SIOL group were bilaterally implanted with Tecnis ZA9003. Visual acuity, stereoscopic vision, contrast sensitivity, spectacle independence, depth of focus, binocular aniseikonia, presence of photicphenomena, and patient satisfaction were assessed at 12 months following implantation.Result:Under 10cd/m2 and 100cd/m2 ,there was significant different in the mean binocular distance , intermediate and near visual acuities between two group. Under high light or low light, the uncorrected visual acuities in MIOL group at 63cm and 100cm distance were signnificantily higher than those in SIOL group(t=8.6236,P=0.0000;t=5.2350,P=0.0000)/(t=6.8571,P=0.0000;t=3.7429,P=0.0006). Under high light or low light, the uncorrected visual acuities in MIOL group at 40cm and reading distance were signnificantily higher than those in SIOL group(t=8.2070,P=0.0000;t=5.5832,P=0.0000)/(t=7.5056,P=0.0000;t=3.1599,P=0.0031). The MIOL group was helpful with reseted stereoscopic vision and decreaced binocular aniseikonia. Binocular sight in two group contrast sensitivity was not significantly different. MIOL group had wider depth of focus than SIOL group. Under 10cd/m2 and 100cd/m2 , there was significant different in reading speed between two group(t=2.6755,P=0.0109;t=2.4767,P=0.0178). In the MIOL group, 90% patients did not need to wear glasses. And MIOL group had higher satisfactory. Conclusions: Combined implantation of refractive and diffractive multifocal lens in both eyes could provide a satisfactory vision at a full range of near, intermediate and far distance to a certain extent, and decreace the needing of wear glasses. Thus improving the patient quality of life.
