| Periodontitis is a destructive disease of the periodontal tissues. Chronic periodontitis has been reported to affect over 30% of the adult population,with severe disease reported in 7%-13%of adults. Periodontitis is not only the leading cause of adult tooth loss,but also be closely related with a variety of systemic diseases. Diabetes which is a common endocrine and metabolic diseases, is often accompanied by a variety of chronic complications, resulting in many organs and multi-system injuries has been confirmed related with periodontitis. It lead to a high rate of morbidity and mortality,and is the third largest disease that endanger human health just following the tumor and cardiovascular and cerebrovascular disease. Periodontal disease is regarded as the complication of diabetes. Nevertheless, no conclusion has yet been made on the mechanism of interaction between the two diseases.The microorganisms and their metabolic products are the initiating factors of periodontitis.However, the excess inflammatory reaction immune response is the leading cause of periodontal defect. In addition,scholars have discovered that the subgingival floar of type 2 DM patients with chronic periodontitis are similar to these of no-DM chronic periodontitis patients ,the amount of pathogenic germs is fewer in type 2 DM patients with chronic periodontitis.Objective:This study compared the differences of the levels of IL-6 and the apoptosis rate of PMN between type 2 diabetes patients and the healths,to investigate whether the PMN of type 2 diabetes patients possess hyper-reactive inflammatory response trait. And we study the effect of actinobacillus actinomycetem on the secretion and apoptosis rate of polymorphonuclear leukocytes in type 2 diabetes patients,which could determine the role of PMN in type 2 DM patients with chronic periodontitis.Methods:Aa were grown under standard anaerobic culture conditions, and the bacterial inocula were harvested, and sonicated bacterial extracts were prepared from the bacterial pellet by centrifugation. Peripheral PMN from healths and type 2 diabetes patients were isolated by Percoll gradient centrifugation. The PMN were stimulated with filtrate of ultrasonic pulverization from Aa as experiment group. As control group, PMN suspension was incubated with PBS. The release of IL-6 was measured at 20, 40, 60 min by ELISA technique. The apoptosis rate of PMN was tested by Flow Cytometry.Result:Incubated with filtrate of ultrasonic pulverization from Aa, the PMN of type 2 diabetes patients released significantly higher levels of IL-6 compared with the healths(P <0.001).The apoptosis rate of PMN from healths was higher than that from type 2 diabetes patients (P<0.001).Regardless of body condition, interaction with filtrate of ultrasonic pulverization from Aa could induce the seretion of IL-6 and reduce the apoptosis rate.Conclusion:1. The sonicated bacterial extract which was prepared from Aa contain most of inflammatory factors could avoid the omission caused by indeterminateness of the thallus. It was an ideal means in researching the virulence factors of periodontal pathogens.2. Periodontal pathogens such as Aa could delay the apoptosis of the PMN and result in the growth of level of inflammatory factors.3. Because of the HG environment, the PMN of type 2 diabetes patients possess hyper-reactive inflammatory response trait. Meanwhile,regardless of stimulating from sonicated bacterial extract, the PMN of type 2 DM patients have a lower rate of apoptosis and could release higher level of IL-6 than the healths, demonstrating that PMN plays a important role in the initiate of periodontitis. |
