Intussusceptions are one of the most common acute abdominal diseases in pediatric surgery. If not treated in time, it could lead to intestinal obstruction and the consequent electrolyte disturbance, acid-base imbalance, as well as infection, poisoning, even intestinal necrosis, dehydration and shock. Therefore, timely and effective treatment of intussusceptions has been a key research project in pediatric surgery. The current treatment methods include conservative treatment and surgical therapy. The former refers to air enemata while the latter includes operative reduction and intestinal resection-anastomosis. Since patients with infantile intussusceptions are mostly under the age of 3, with consideration of physical fitness in children and their tolerance of operations, conservative treatment is the first choice. The conventional reduction treatment with air enema has to be carried out under X-ray inspection. It is easy and simple. Nevertheless, the patients have to be exposed to X-ray for a long time during the reduction process, which will affect their growth and development and blood systems to a certain extent. By contrast, reduction by the pressure of decoction is an effective way to avoid such radial damage to the patients.OBJECTIVE:To increase knowledge of the key operation points, clinical values of diagnosis and curative effects of enema with traditional Chinese medicine under type-B ultrasonic inspection for treatment of intussusceptions in infants; to evaluate the effect of Jianpishunfutang in restoring intestinal functions of patients. PATIENTS&METHODS:100 cases of pediatric intussusceptions were divided into a treatment group of 50 cases and a control group of the other 50. For the treatment group, give fluid infusion and sedative spasmolytin before administering enemas with Chinese traditional medicine under type-B ultrasonic inspection. For successful cases, give fluid infusion and anti-infection treatment, and give oral activated charcoal to evaluate effects. For the control group, carry out air enema under X-ray inspection after giving fluid infusion and sedative spasmolytinRESULTS:47 of 50 cases of the treatment group achieved successful reduction; the success rate was 94%.2 of the failed cases were successfully treated with air enema afterwards, and the last 1 needed further operation, but there was no resection due to intestinal necrosis. After confirmation of successful reduction under type-B ultrasonic inspection, give these 47 patients further fluid infusion, anti-infection treatment and supportive treatment. They were given 5ml of liquid paraffin with 0.5-1. Og of powdered activated carbon. The activated carbon was found in the excrement within 4-6 hours in 45 cases, and the other 2, within 8 hours. These 47 patients were discharged after remaining for 2-3 days in hospital under observation.CONCLUSION:Enema with traditional Chinese medicine (Jianpishunfutang) under type-B ultrasonic inspection not only can reduce intussusceptions by the pressure of decoction, but also can help restore intestinal functions through absorbance of effective elements of Chinese traditional medicine. In addition, it can avoid the radial damage of air enema under X-ray inspection. It is simple, safe, and worth promoting for clinical application.
