Background:Chronic obstructive pulmonary disease(COPD) is a disease state characterized by airflow limitation that is not fully reversible. The airflow limitation is usually both progressive and associated with an abnormal inflammatory response of the lungs to noxious particles or gases. The Patients of chronic bronchitis, pulmonary empHysema whose spirometry show airflow limitation not fully reversible can be diagnosed as COPD. Patients of COPD, who are seriously affectted the labor force and quality of life,usually have chronic cough, sputum production, air hunger or dyspnea, asthma and chest distress. The medicine workers pay more and more attention to COPD, which is the common and frequently-occurring diseases in respiratory and is NO.4 in the causes of death in the current,owe to a great deal of COPD patients, high case fatality rate and weighty economy burden. This article summarize the recordation about COPD in the archaic Chinese medicine literature, the progress of modern Chinese medicine in the study on COPD, the canonical prevention and cure for COPD, progress of modern medicine in the study on COPD, the connection between hemorheology and COPD, and so on. On the basis of the above summarization, we probe into the curative effect on AECOPD of clearing away heat, resolving pHlegm and promoting blood circulation in this antitheses, prospective study by measuring the artery blood gas analysis and blood hemorheology before and after treatment so as to provide scientific evidence for treating COPD with the management of clearing away heat, resolving pHlegm and promoting blood circulation.Objectives: The aim of the study was to assess the clinical effect of clearing away heat, resolving pHlegm and promoting blood circulation on Acute Exacerbation Chronic Obstruetive Pulmonary Disease (AECOPD) by differentiation of pHlegm, heat and blood stasis, which affected clinical symptoms, sign, artery blood gas analysis and blood hemorheology. The result may offer guides in curing AECOPD with the management of clearing away heat, resolving pHlegm and promoting blood circulation. At the same time, we can preliminary discuss the area of side effects in the treatment of chronic obstructive pulmonary disease by Chinese medicine.Methods:Sixty hospitalized AECOPD patients (StageⅠ-Ⅲ) by differentiation of pHlegm, heat and blood stasis are randomly divided into two groups, the trial group and the control group. The patients of trial group take the Chinese medicine decoctions and injections which have the efficacy of clearing away heat, resolving pHlegm and promoting blood circulation, and simultaneously accept conventional therapy of the western medicine same as the control group (controllability oxygen treatment, bronchodilators, antibiotic, expeetorants). The period of treatment is 7 days. To compare the result of two groups by analyzing clinical symptom, sign, artery blood gas analysis and blood hemorheology. The safety index is blood analysis, urine composition, stool(include occult blood), liver function and renal function.Result:1. Clinical symptom and sign:After the treatment, melioration of clinical symptom, such as cough, the volume of sputum, the properties of sputum, the difficulty of expectoration, dyspnea, appetite, thoracodynia, and sign,such as wheezing rale and moist rale, in both groups have significant difference(P<0.01 or P<0.05). Compared with the control group, melioration of the trial group, such as cough, the volume of sputum, the properties of sputum, the difficulty of expectoration, dyspnea, appetite, wheezing rale and moist rale is greater, showing significant difference (P<0.01 or P<0.05). The above result show that the therapy of two groups both can help AECOPD, and the trial group is better than the control group.2. Artery blood gas analysis:After the treatment, the melioration of PaO2 and PaCO2 of each group have significant difference(P<0.01). Compared with the control group, melioration of the trial group is greater, showing significant difference(P<0.05). The above result show that the therapy of both groups can help to improve the indexs of blood gas analysis, and the trial group is better than the control one.3. hemorheology:The whole Blood viscosity, include high cut, middle cut, low cut, hematocrit, eythrocyte electropHoresis time, erythrocyte aggregation, rigidity index and plasma viscosity of treating group have significant difference (P<0.01 or P<0.05) after 7-day treatment compared with pre-treatment. After the treatment, the whole blood viscosity (high cut), plasma viscosity, hematocrit and erythrocyte aggregation, improve more greatly in the treating group(P<0.01或P<0.05). The above result show that the therapy of both groups can help to improve the indexs of hemorheology, and the trial group is better than the control one.Conclusion:The management of clearing away heat, resolving pHlegm and promoting blood circulation, the curative effect of which was better than the control group, could improve the clinical symptoms and sign, remedy the state of anoxia and carbon dioxide retention, hemorheology so as to accelerate rehabilitation of AECOPD patients.
