Objective:To investigate the risk factor of cardiac arrhythmia following operation of lung cancer and search for the prophylaxis of postoperative cardiac arrhythmia.Methods:With methods of case-control study. an epidemiological survey of the cardiac arrhythmia following operation of lung cancer in Cancer Hospital of Guangxi Medical University during 2004,4-2009,4 was conducted. Arrhythmia group compared with the non-arrhythmia group differences in factors, univariate and multivariate Logistic regression was performed, analysis of risk factors for postoperative arrhythmias, assessment of postoperative arrhythmia and other common risk factors of relevance.Results:In 419 lung cancer patients, the post-operation cardiac arrhythmia occupied 12.4% (52/419) of all the cases. Arrhythmias usually occur in the first three days afte operation, in which atrial fibrillation accounting for 40.4% is the most common, and the following is atrial extrasystole and the ventricular extrasystole each accounting for 25%and 21%. After preoperation, postoperation and tumor itself four aspects studied, the 25 potential factors analyzed, results showed that older than the year of 65,smoking history, preoperative pulmonary function abnormalities, having arrhythmia and chronic pulmonary emphysema histories, lung surgical resected, intraopertive, package with injury, postoperative electrolyte and acid-base imbalance, Postoperative complications (atelectasis, fluid, pneumothorax, pulmonary infection), no postoperative mana-gement, pathological stage late groups, the postoperative arrhythmia were significantly higher on the incidence of postoperative arrhythmias than matched group(p<0.05). Meanwhile, Risk factors were identified by multifactor noconditional logistic regression, age older than 65y, tobacco smoking, poor pulmonary function, pericardial injury, acid-base imbalance, electrolyte disorders, large scale of clearance of lymph nodes, pathological grading, complications after operation. The protective factor was postoperative analgesia.Conclusion:Age (≥65 years), smoking history, history of preoperative arrhythmias, history of emphysema, total pneumonectomy,postoperative electrolyte imbalance, acid-base balance, postoperative complications (atelectasis,pneumatothorax,massive introthoracic hydrops) and late pathological grading were risk factors for postoperative arrhythmias, The postoperative pain is a protective factor. Arrhythmia predilection time is the first three days after lung cancer surgery, we should acute treatment cardiopulmonary complications, improvement preoperative cardiopulmonary function, strengthen the monitoring and preventive treatment.
