Part 1 Established rat model of diabetic burn[Objective] established the Wistar diabetic rats model,and in its surface produce 10% of their body surface area deepⅡdegree burns.[Methods] Making the diabetic rat model by intraperitoneal injection of STZ.With control water temperature and burn time to make their way deepⅡdegree burn model.[Results] Blood sugar in diabetic rat model are stable, and concentration is greater than 16.7mmol/L, producing the syndrome of diabetes.The results of immunohistochemistry and HE staining show that the secretion of pancreatic islet cells are reduced and some cells have been destroyed. Modeling of deepⅡdegree burns can be copied easily.HE staining showed the skin surface is damaged but true subcutaneous tissue are integrity. Residual glandular epithelial cells formate the epithelial islets.[Conclutions] Compared with other methods used to make diabetes model, Intraperitoneal injection of STZ is with good repeatability,and experimental procedure is simple, convenient, reliable. The operation is easy to quantify and the process standardize simply,and the model is also stability.The materials of Controlling water temperature and burn time to make deep II degree burns are simple,and processes aer easy to operate. Repeatability and stability are the advantages of the methods.Part 2 In the wound healing process,Locally administered insulin and rhEGF affect the matrix and the role of cvtokines.[Objective] Exploring the RhEGF combination of insulin how to affect the matrix and the role of cytokines during the burn wound healing process in diabetic patients, and the relationship between the action and wound healing.[Methods]①Diabetic rat model of burn debridement after randomly divided into four groups:Control group (A group), wound using saline; Insulin group (B group), wound is the infiltration of local injection of insulin; Spraying the rhEGF group (C group), wound Spraying the amount of rhEGF; Combination of insulin and rhEGF group (D group):wound applied with the same dose Insulin of group B, and the same dose rhEGF of group C. After you run out of drugs on the wound, dressing on the wound.②We test the OHP content (Sample alkalinehydrolysis),TGFa content (in situ hybridization) and VEGF levels (ELISA) at different time points In the wound healing.③In the wound healing process,with the same time point we detect rate of wound healing in each group and the distribution of VEGF (anti-VEGF antibody immunohistochemistry).[Results]①The rate of wound healing in Group B and Group C has no significant difference at different time points. The wound healing rate in Group D is higher than the other three groups at different time points.②OHP levels of Group B, C, D at different time points were higher than the Group A,and OHP levels of Group D, B, C at different time points are different (P<0.05).③TGFa levels in Group B, C, D has the right to improve, significantly lower expression of the control group.The expression of Group D at different time points are different from Group B, C (P<0.01).④VEGF is mainly distributed in vascular endothelial cells and wound at the base. Group D,to maintain a longer time at a high level, has differences between group B,C (P<0.05).The concentration of maintaining and the peak times appear in Group B, C, D were better than group A.The difference between Group B and Group C was not significant.[Conclutions]①With insulin to control blood sugar in the local case, the external rhEGF can significantly accelerate the healing of diabetic burn wound.On the one hand this may be added the lack of wound-stimulating factor in itself, on the other hand the control of blood sugar can regulate receptor expression to promote healing.②Thorough debridement and enswathement can increase the contact time for rhEGF.Local infiltration injection of insulin also decreased the volatile, continuing to function,and provide a good environment for rhEGF.③Combination rhEGF and insulin in diabetic wound healing process,can coordinate regulation of collagen metabolism in wound healing.④After the local blood sugar control, rhEGF stimulation can increase the expression of other cytokines and accelerate wound neovascularization, re-epithelialization, wound closure as soon as possible.coordinating and promoting wound healing.⑤Diabetic wound difficult to heal may be associated with high glucose, the expression of certain cell factor receptor is inhibited or too low, then epithelialization and angiogenesis disorders are related. In addition, there are many other aspects.
