Background: LV (Left ventricular) remodeling after AMI (acute myocardial infarction) refers to alterations in ventricular mass, chamber size, and shape. At the cellular level, myocardial pathologic changes involve myocyte hypertrophy and fibroblast hyperplasia accompanied by an increase in collagen deposition within the interstitium.Accumulating evidences showed that the cardiac RAAS (rennin-angiotensin-aldosterone) system was actitivated during the LV remodeling process that occured after AMI. Aldosterone expressed in the noninfarcted area and played an important role in the remodeling. The phenomenon'aldosterone escape'had been explained. RALES and EPHESUS showed that the addition of an aldosterone blocker to the optimal medical therapy reduced morbidity and mortality among patients with AMI complicated by left ventricular dysfunction and heart failure. Both clinical tries showed that the block of aldosterone was benefit for the host.
