Objective: Many studies showed that HHcy is the independent risk factor ofcerebral vascular disease, some researchers consider the HHcy can make the cerebral vascular instability, then lead to the sudden accidents. So I design this research to study the association between homocysteinemia and intracranial vascular stenosis in patients with cerebral infarction. Methods: 83 patients with cerebral infarction admitted from Oct 2008 to Jan 2009to Qilu Hospital of Shandong University were chosen as our participants. They were all accord with the standard : (1) accorded to the diagnose standard of cerebral vascular disease that was revised in 1995,and were confirmed by MRI. (2) Age > 18,and had detailed clinical data. (3) the time from the accident happened to the patients admitted to the hospital must less than 14 days.(4)the patients and their family agreed with the study. They were classified into stenosis group (55 patients) and the normal group (28 patients) based on MRA.2ml elbow vein blood was withdraw at fast state in the morning. After the sample were collected in EDTA test tube, centrifugated within four hours ,and then the plasma was taken out, serum homocysteine was measured with fluorescence biochemistry quantitative assay. The kits were provided by Jinan XingEn Biotech Limited Company .Batch No: Hcy—07 Exp.Date:Feb.08. At the same time ,we detected the other common risk factors, like age, sex, the serum concentration of glucose, total cholesterol(Cho),triglyeride(TG),low density lipoprotein(LDL),high density lipoprotein(HDL),apolipoproteine-Al(ApoAl),apolipoproteine-B(ApoB),and the ratio of ApoA1 and ApoB. Then , We compared the correlated factors between the two groups.Results:(1)The level of serum Hcy in stenosis group was higher than the normal group(P= 0.009<0.05). The percent of Hyperhomocysteinemia in stenosis group was significantly higher than that in normal group(x~2=6.019,P<0.05).(2) There was significant difference in the concentration of Cho between the two groups(4.64±0.75,4.24±0.66;P=0.018<0.05).(3) Based on multivariable stepwise Logistic regression model ,the odds ratio of homocysteine was 1.146(95%CI:1.021~1.287,P=0.021) and Cho was 2.406(95%CI:1.145~5.055,P=0.021) with additional adjustment for convention risk factors. Conclusion: It suggested that high level of plasma homocysteine is an independent risk factor for intracranial vasculars stenosis.
