Research Background:Early detecting the artery's function using the two-dimensional ultrasound echo tracking technology and color Doppler technology to undergo research on vascular morphology and blood theology,Early atherosclerosis is characterized by the pathological cell infiltration,smooth muscle cell proliferation and intimal migration of arterial and cardiac function and occurrence of subtle interplay of changes in the application of the current conventional ultrasound imaging detection methods make it difficult to judge.due to the mutual influence between heart and vascular system,we need to analyze them comprehensively when evaluating their respective functions.WI(wave intensity) technology recently is a new measurement method for hemodynamic index,and it is a kind of real-time measurement system based on traditional eTRANKING technology.Different from the past detecting techniques,WI can reveal the interaction mechanism of artery and heart,analyze arterial function,and provide the systolic and diastolic functions of patients' heart.WI technology uses the linear relationship between blood vessel diameter and pressure by tracing the endometrium,combines the color Doppler technology,analyze the wave size of forward wave and backward wave,and thus can evaluates functions of heart and artery quantitatively,comprehensively and accurately.Objective:Detect the carotid artery endothelial functions of diabetics through WI technology,and provides more objective and accurate parameters to refer to for clinical to screen,follow and evaluate the intervention treatment's effect for diabetics' macroangiopathy.And further explore the application value of WI technology on non-invasive evaluation of hemodynamic performance and relationship between heart and vascular system.Subjects and Methods:In March 2008 to July 2008 the Eleventh Five-Year Project and long-term follow-up ultrasound in patients with diabetes,rule out the possibility there are 11 cases of carotid artery plaque,and seven cases of abnormal cardiac function,the successful detection of 173 cases,case 87,patient with diabetes and hypertension (the diagnostic criteria is in line with that designed for diabetes and hypertension by the World Health Organization in 1999.),and case 91, ordinary people whose age,gender and body mass index match the above patients.The selected patients have one or more years' disease course,and rule out whose carotid arteries are affected significantly by secondary hypertension,severe lever,kidney and cardiovascular and cerebrovascular diseases,metabolic disorders,etc.Use Aloka10 color Doppler ultrasound diagnostic apparatus to detect subjects' carotid intima-media thickness IMT,and use WI techniques to detect the related hemodynamic indices,including forward systolic wave's peak W1, forward expansion wave W2,stiffnessβ,pressure strain elastic coefficient Eρ,compliance A C,velocity of pulse wave conduction PWVβ.Result:FSB,PP was obviously increased in diabetes group and diabetes with hypertension group than the control group.W1:was increased in diabetes group than the control group,and diabetes with hypertension group was obviously increased than the diabetes group. W2:there is no significant different in diabetes group and the control group,and diabetes with hypertension group was increased than the diabetes group and the control group.IMT,β,Ep and PWVβwere Gradually increased in the control group,diabetes group,diabetes with hypertension group,while AC reduce,there is significant different.W1 and W2 are the independent variables,and IMT is the dependent variable for single-factor correlation analysis,and the correlation coefficient r is 0.226,0.214,P<0.01,W1 and W2 are the independent variables, and IMT is the dependent variable for single-factor correlation analysis, and the correlation coefficient r is 0.211,0.191,P<0.01.Conclusion:WI measurement system evaluates arterial wall's flexibility and heart's function comprehensively through measuring the arterial diameter(systolic and diastolic) and the change of arterial blood flow velocity.This technology has many advantages,such as original data's real-time collection,good reproducibility,non-invasive,etc. which provides a new accurate and rapid method for detecting atherosclerosis objective basis for clinic to screen,follow-up,and evaluate the treatment,thereby guides treatment and prevention.
