With the development of economic growth and people' living standards, medical services are highly demanded. At present, the existing problems of public hospitals are inadequate management system and reimbursement mechanism, lacking-of-vitality operational mechanism. Developing private medical institutions helps to break the marketing monopoly by the public hospitals, establish legal person administration structure with distinct right-duty relationship, make up for the lack of financial input and accelerate the reform of pernonnel system, bonus allocation and financial management. The equity of health care market should be improved by our government in order to create competitive environment and improve the operating efficiency.This article takes the development of private medical institutions as the research project during the process of the establishment of modern health care market in Shanghai. It makes a general analysis of the problems of the development of private medical institutions in China and its possible solutions. Combining some relative theories of medical products and market, the article elaborates on the necessity of developing modern health care market and private medical institutions. Then the article introduces the development, characteristics and management experience of private medical institutions throughout the world and China and analyzes the history, policies and current situation of the development of private medical institutions in China and Shanghai. The article emphasizes the problems of the development of private medical institutions in Shanghai and its causes. The existing problems of private medical institutions in Shanghai are small scale, low rations of medical insurance contracting institutions, low quality, low efficient allocation of resources and illegal practice. The main reasons that restricting the development of private medical institutions in Shanghai are the following: impertinent standard admission, the impact of regional health planning and the layout adjustment of health care resources, unfair policy of medical insurance contracting institutions, imperfect tax and supervision policy, personnel bottlenecks. All of these reasons lead to the situation that private medical institutions can not enjoy the same policies as public medical institutions enjoy in Shanghai.Based on the above analysis, the article proposes five suggestions to improve the development of private medical institutions in Shanghai. The first suggestion is that the accreditation system should be improved. The second suggestion is about expanding private medical institutions into the scope of medical insurance contracting institutions. The third suggestion is about reasonably adjusting the tax policy. The fourth suggestion is about the supervision and management of private medical instituions. The last suggestion is that the health personnel management system should be reformed gradually.
