The Relationship Of Augmentation Index And Carotid Artery Intima-media Thickness With Chronic Kidney Disease Patients

Objective:To study the affecting of augmentation index(AI) and carotid artery intima-media thickness(IMT) in patients with chronic kidney disease.Methods: Part one:one hundred maintenance hemodialysis(MHD) patients (47 female and 53 male),were included in the present study.Their mean age was 51.2±13.7 years old.And fifty-two healthy people (24 female and 28 male) as the normal control,matched with age,sex,body height and body weight, were chosen.Their mean age was 50.5±14.8 years old.AIX75bpm was evaluated by HEM-9000 AI type (Japan omron corporation) pulse wave detection device, whose heart rate was corrected by 75 times per minute.Pearson's correlation coefficients were used to analyze the associa- tion of clinical parameters.Stepwise regression analysis was employed to evaluate the independent parameters relating to AI.Part two:Fourty chronic kidney disease(CKD) patients (15ml/min/ 1.73m2≤eGFR <90 ml/min/1.73m2 ),whose mean age was 50.0±11.9 years old (20 female and 20 male),were included. And fourty healthy people as the normal control, matched with age,sex,body height and body weight, were chosen.Their mean age was 49.9±14.7 years old.AIX75bpm was evaluated by HEM-9000 AI type(Japan omron corporation) pulse wave detection device,whose heart rate correction was 75 times per minute.IMT was evaluated by color doplor ultrasonography.Pearson's correlation coefficients were used to analyze the association of clinical parameters. Stepwise regression analysis was employed to evaluate the independent parameters relating to AI. Results: Part one:AIX75bpm was higer in MHD patients than in healthy people (P<0.01),as well as thier blood pressure(P<0.01),which included systolic blood pressure,diastolic blood pressure,mean blood pressure and pulse pressure,after adjusting for gender,age,height,weight and body mass index (BMI).Pearson's correlation showed that,AIX75bpm was positively correlated with age,mean arterial pressure(MAP) and smoking,while negatively correlated with height,weight,BMI,hip circumference. AIX75bpm was higer in femal patients than in male patienrs ,but there was no significance after adjusting other factors (P>0.05),such as age,MAP, height, weight,BMI,waist and hip circumference.Multiple stepwise regres- sion analysis showed that,age,height and mean MAP were independent deterni- nants of AIX75bpm in these people .Part two:In CKD patients, AIX75bpm was higer than that in healthy people (P<0.05), as well as thier blood pressure (P<0.01),which included systolic blood pressure, diastolic blood pressure,mean blood pressure and pulse pressure,after adjusting for gender,age,height,weight and BMI. Pearson's correlation showed that, AIX75bpm was positively correlated with age,mean arterial pressure(MAP)and smoking,while negatively correlated with height,weight,BMI,waist and hip circumference.Multiple stepwise regression analysis showed that,height,BMI,and MAP were independent deterninants of AIX75bpm in these people. Hyperlipidemia, including total cholesterol, triglyceride,LDL-cholesterol, HDL-cholesterol, had no statistics significence effect on AI. While ACEI/ARB,CCB had the effection on reducing the augmentation index,and also had no statistics significence. IMT was higher than that in healthy people (P<0.001),and pulse pressure (PP), proteinuria and age were its independent deterninants in multiple regression analysis. IMT and AI correlated to each other (r= 0.339,P= 0.032).Conclusion: Part one: AI was higer in MHD patients than in healthy people.Age, height and mean arterial blood pressure were independent deterninants of AI.Part two: AI and IMT was higer in CKD patients than in healthy people. Height, BMI and mean arterial blood pressure were independent deterninants of AI.Pulse pressure (PP),age,proteinuria were independent deterninants of IMT. IMT and AI correlated to each other.
