Various anatomic complications of pulp cavity and canal system, the pathological changes of the root apex before treatment, the differences of medical instrument and drugs, technique level between dentists, all the causes lead to this situation that there are still some failed cases of endodontics treatment though all the clinical operations are done in aseptic condition. The causes for the failure are various, for example, resinifying therapy, pulp mummification, overfilled and underfilled canals, root apical diseases, missing canals, and so on. Sometimes the root canals have clinical symptoms, sometimes the root canals don't have any clinical symptoms and they just need to have the improved root canal filling before Prosthodontic treatment. The successful key for the endodontic retreatment is analyzing the causes of failure, the former treatments, the situation of diseased teeth and selecting the correct ways of endodontic retreatment. Furthermore, with the modern root canal therapy and instruments are introduced, various special operative microscope and ultrasonic device are used in the endodontic retreatment, the successful rate is increased. Even if most of the hospitals don't have the operative microscope and ultrasonic device, the chances are also greatly increased if we have the reasonable treatment, cautious clinical operation, right drugs and instruments. So we should try our best to take the conventional endodontic retreatment for the failed cases. If the conventional retreatment fails, apical surgery is recommended.The study, based on clinical cases of endodontic retreatment, includes the reasons and the effective treatments, and we hope the findings and treatment options will be further value in the clinical operations.
