Experimental And Clinical Studies On The Buzhongyiqi Tang Combined With Imodium In The Treatment Of Defecation Obstacles After Colectomy

Objective:①Experimental study:Build the mode of defecation obstacle after collectomy, then observe the activity change of endocrine cells in colon mucosa whether the Buzhongyiqi tang combined with imodium can influence the activity of colon mucosa cells.To investigate the intrinsic mechanism underlining the defecation obstacle after colectomy and functional mechanism of Buzhongyiqi tang combined with imodium so as to provide a new ideal and method to intervene the defecation obstacle after the colectomy.②Clinical study:To investigate the therapeutic effection of integrative medicine for defecation obstalce after colectomy through the observation of influence exerted by Buzhongyiqi Pill combined with imodium administered to patients after colectomy.Method:①Experimental study:60 Chinchilla rabbits all received colectomy,then divided randomly into four groupsxontrol group which received no special management, modern medicine group recieved imodium through intragastric adminstration,traditional Chinese medicine group received Buzhongyiqi tang through intragastric administration,integrative medinice group received Buzhongyiqi tang combined with imodium through intragastric administration.After two weeks of treatment,samples at anatomotic partof colon excised whose 5-Hydroxytryptamine(5-HT),ChromograninA(CgA) positive-staining cells at mucosa membrance were detected by the immunohistochemical method.②Clini cal study:80 patients who conformed to the including criteria were divided randmly into two groups.Before treatment was initiated all patients' defecation function were evaluated accoding to the table of defecation function evaluation.The control group recieved routine management,and the experimental group recieved Buzhongyiqi Pill in addition to routine management.After one week all patients' defecation function were evalua -ted again.Result:①Experimental study:The number of postoperative 5-HT positive-staining cells in preoperative group and control group,modern medicine group,traditional Chinese medicine group,integrative medinice group were 10.40±2.22,26.27±2.35,18.20±2.71, 15.93±2.46,14.33±2.87 respectively;The number of CgA positive-staining cells in post-operatiegroup, control group,modern medicine group,traditional Chinese medicinegroup,integrativemedinice group were 20.60±5.34,51.51±6.13,36.10±5.34,28.36±7.16,29.33± 8.17 respectively;there was high expression of 5-HT,CgA positive staining cells at the postoperative group compared with the preoperative group,where there was a significant statistical differences(P＜0.01).There was relative low expression of 5-HT,CgA positive stainig cells at integrative group compared with other groups,which has a significant statistical differences(P＜0.01).②Clinical study:The effective rate of testing roup was 85%and control group was 55%.The defecation functionin testing group were improved more significantly than those in control group(P＜0.01).Conclusions:①Experimental study:The expression of 5-HT,CgA positive-staining cells would be stimulated by the colectomy and depressed by the Buzhongyiqi tang combined with Imodium.②Clinical study:The defecation obstacle after colorectal cancer surgery would be improved with the treatment of the Buzhongyiqi pill combined with imodium.
