Objective To evaluate the relationship of preoperative serum CA125 level with clinical-pathological features and survival in patients with epithelial ovarian carcinoma.Methods A retrospective review of 279 patients with epithelial ovarian carcinoma in our hospital was performed. Serum CA125 levels were assessed (Magnetic Antibody Immunoassay ) for their relationship to histologic type, FIGO stage, tumor grade, presence of ascites. And its association with the outcome of first-time cytoreductive surgery and survival was analyzed through follow-up data.Result A total of 279patients were included in the analysis. The median CA125 value for all patients was 339.2 U/ml (range 3.6–20220 U/ml). High preoperative CA125 values did not correlate with increasing age or high tumor grade, but were found to be significantly associated with serous histology and other histology other than mucous / clear cell carcinoma; advanced(II~IVstage) compared with early (stageI) disease, the presence of ascites compared with absence of ascites. And the difference between mucous/ clear cell cancer and serous/other histology was also found both among early and advanced diseases. However, pre-operative serum CA125 level alone is not reliable to predict the surgical outcome of primary cytoreduction or prognosis, and we found that only stages and the course of chemotherapy were independent prognostic indicators for epithelial ovarian cancer.Conclusion The ability of preoperative serum CA125 to reflect the tumor load and disease progress might be established in serous and other histology other than mucous and clear cell carcinoma. However, it remains questionable for its role of predicting the outcome of primary cytoreductive surgery or prognosis.
